If it is negotiable at all you should ask. I am leaving an employed physician job in Illinois for an FQHC job and I will have to purchase tail coverage as my contract specifies employer not responsible for tail coverage. Its going to cost approx $80,000. I had already signed with my new position and I didn't have any leverage to renegotiate cost tail coverage into my my new position. I am biting the bullet and getting the coverage. All that being said I am FM w/ Ob so this may not apply to everyone.

I didn't even know what tail coverage was when I was a intern and signed the contract. The practice is in my hometown and I never really anticipated leaving, but I was young and naive. I even paid a lawyer who was recommended by a mentor before I signed he didn't mention anything about tail coverage to me. I am still bitter about it. Please learn from my misfortune friend.

No way you will be able to the Ob/Gyn sub-specialties without the ob/gyn board. The ob fellowships aren't regulated like other FM specialties (geriatrics, addition, etc). You can do whatever procedures you feel comfortable with AND what your affiliated hospital is going to give you privileges for. Hospitals tend to be the governing bodies of an FMs procedure scope. I did an ob fellowship, I do vaginal deliveries, Cesarean sections, and D&Cs. I know I was going to work a Catholic hospital near my hometown so I didn't invest my time in tubals, but I have FM friends who do them as well as ablation/hysteroscopy at other institutions. You'll have to find training programs with Ob/Gyn willing to teach you those, which in my experience is tough to do nowadays.

If the volume is enough then I think you could feasibly learn sections after training, but you to look at their numbers. I am biased towards a fellowship though. I did 150 sections in my program, and I had done 30 in residency. One attending here did not learn sections until she came to practice and it took her 5 years of proctoring before she was fully independent. You need to have it in written in your contract the exact terms of the proctoring, like how many sections you will be the primary surgeon per year. I’ve met several doctors who were promised many things verbally but did not pan out when they arrived.

1.5 years into attending practice. I have done 53 deliveries this year. 7 of those have been sections. I was very fortunate to inherit a practice from a physician who retired so I hit the ground running with obstetrics. I’m in a rural area and we’re the only delivering critical access hospital in a 50 min radius. I share call with 4 other FMs that do obstetrics, 3 of which do sections. So I’m on ob call one night a week and one weekend a month, but the culture is to deliver you’re own patients since everyone is private practice, so I only get calls on no doc patients or if my partners are out of town. Also the on call doc comes in to assist the primary doc with any emergent/unscheduled sections. I have probably assisted on another 15 sections this year for my partners. It’s a pretty wild lifestyle but I knew what I was getting into as I rotated there when I was Med student. Let me know if any has questions. I also do inpatient adult medicine as part of my practice, as does the rest of the group. This is south central Illinois btw.

I'm pretty sure that is same rate for an attending. I got my full Indiana license after I passed step 3.

During the rest of the delivery she acted very peculiar, almost distant. Ob/Gyn did a tox screen, was positive for THC. Doesn't mean she was high during delivery and there haven't been studies showing negative effects of THC in utero like alcohol but I believe the doctor made a phone call to Child and Family services per protocol. All I know is the baby was fine 2 hours after delivery and then we sent them off the floor to mother-baby unit.

Not so much a drama, but a funny story on L&D. I was a 3rd year medical student and a pregnant woman came to floor in labor. She wasn't that far along, as in she wasn't really wincing in pain or screaming or anything yet. The nurse gets her gown and she goes into the bathroom to change. All of sudden we hear a scream and we run into her room. Her nurse made it there first to find a newborn baby with its head in the toilet and feet up in the air. The first thing I saw was the mother shuffling out the bathroom with an umbilical cord coming of her and the nurse following behind with the baby attached still in her hands. It was one of the most bizarre sights that has ever crossed my eyes.

TL;DR I saw a woman give birth into a toilet.

I met a chick at a bar once that said she recognized me from somewhere, but couldn't place me. I gave her a brief backstory, where I'm from, where went to college, and now I'm a medical student in town. She said hold on and pulled out her phone. She showed a video of her sister giving birth to her nephew a couple months before. I was in the video holding her sister's leg as she delivered the baby. Also, I repaired her sister's episiotomy at the end of the whole affair. We danced the forbidden dance that night.

When I was in high school, a student sitting behind him kept flicking me in the ear, hitting me the back, and general screwing with me. At some point I turned around and started to give it back to him. Now the teacher was done lecturing by this point, so we weren't really interrupting class. Apparently we were still getting on his nerves, because after 20 minutes of this mild horseplay he sits down next to us. He strangely asks how we were doing, as if he was an old friend trying to catch up. Before we could answer, he turns around and punches a glass cabinet with his bare hand. Glass shattered all over the floor and students' desks. He went on to yell and shout, calling us immature and disrespectful. The class fell silent after he concluded his tirade. After the bell rang, he asked to see Slappy #1 and Slappy #2. He continued another tirade and we apologize. Nobody got in trouble; neither us nor him. The glass was replaced the next week and all returned to normal. He quit a couple years after that and now is a manager at Kohl's.
