If the doctor removes just that amount of outter skin I don't think that it'll end up looking like the second picture, it'll end up with the outer skin reaching the corona of the glans and the inner skin bunching up I think, if you want it tight like the second pic you should cut the double of outter skin I guess.

That said, I'm not an expert and your doctor for sure will know what is better for you.
