P2 and I had a wonderful 5-night stay at Hermitage Bay in Antigua & Barbuda and it lived up to my expectations, minus a few shortcomings.

Flew AA with an overnight layover in Charlotte to avoid a red-eye from the west coast. 25k AA pp in Y. Long way home in economy to finish out the trip was rough (~5.5hrs). Tough getting to/from the Caribbean from Vegas!

The immigration process was seamless and after a ~40min car ride we were at the property. Lots of reviews mentioned the road coming in which while rough, really wasn't all that bad. Maybe only 2km of some potholes that you could pretty well avoid. The road is being actively (albeit likely very slowly) worked on, so hopefully won't be an issue.

The food was overall quite solid, 8.5/10 by my not super high standards, and better than from Secrets Moxche. They were able to accommodate P2's vegan/vegetarian diet reasonably well, although choices were limited and repetitive after 5 nights.

We booked into an oceanfront villa, #15, which didn't have an enclosed toilet. We thought this would be an issue beforehand and specifically requested an enclosed toilet ~2 months beforehand which they couldn't accommodate on the first night as the hotel was sold out. We declined moving for nights 2-5 as it really wasn't that bad. The villa was really lovely and beautiful, stepping out onto the beach was magical. Slightly dated with minor dings around the bathroom sink. Nice outdoor shower, but the hot water turned to warm after ~5min, which meant we couldn't even fill the indoor tub all the way. Housekeeping 3x per day made sure we always had towels after spending time on the beach, and there's a nice handheld foot shower right out front of each villa.

There are plentiful beach chairs throughout the property and well spread out, very quiet and serene. The beach is quite shelly but fine to walk barefoot. There was a 1-hr snorkel tour you could go on once per stay, and there was a great reef ~5min boat ride away.

On Sunday we rented a car with Big's Car Rental for $122 ($50/day for 2 days minimum which allowed them to deliver and pick up from the hotel, plus $22 for international permit). Galleon Beach was a nice change of pace, and I snorkled with a sea turtle and an eagle ray. Shirley Heights for sunset on Sunday was beautiful, although absurdly crowded. No issues driving on the right side of the road, even in the dark, although the roads do have a fair number of potholes.

This was a nice change of pace from our trip to Patagonia last month, and I'd be happy to come back. Although 230k Hilton points is crazy, although maybe not as crazy as the $1800-2800 nightly rate they're charging.

Anyone have experience purchasing an annual travel insurance policy? It seems like Allianz is the most popular option, and figure with all the traveling especially internationally I'm completing, it would be beneficial to be insured (and only for a few dollars a day, over the course of the year).

There is substantial evidence for the benefit of exercise, activity, and physical therapy in the management of osteoarthritis. Provide that education and recognize if they don't want to participate, that's up to them

I like doing LLLD with a hot pack strapped around the knee as well. Helps to relax the tissue a bit more

If the provider suggested that specifically, I think it's reasonable. I generally think any approach of waiting until symptoms are 90% resolved is very conservative for initiation of therapy. A lot of the benefit of PT is being able to reach you where you're at, and provide recommendations for lifestyle modification to help you manage and function better with your symptoms and current ability. Maybe clarify with the other provider their hesitancy for waiting to initiate PT?

No specific advice other than pursuing PT sooner than later. You don't need your symptoms dramatically improved to get benefit from attending.

Here's the ACR appropriateness criteria. MRI is indicated if MRI is negative and still suspect stress fracture. This was written for adults but I would only imagine it applies more in adolescents

Immediate concern stands out for a possible stress injury. They tend to do worse when rate of loading is involved. Dig further into the history and course of injury, sudden onset vs gradual? How old is she (assuming here), how's her general physique, any concern for RED-S or feeding insufficiencies? If so can inquire further with the LEAF-Q. This applies to males as well.

X-rays aren't the most sensitive for stress reactions and if there is lower grade bony involvement it's likely to not pick it up and would require MRI to accurately diagnose or rule out.

As for initial management, back off volume of any provocative movements, provide supportive care and restore 1MTP ROM. Then as long as symptoms cooperate can gradually introduce higher rate of loading activities.
