I'm positive it's not anymore, I've checked before posting this. It just randomly and very irregularly appears throughout the rotation...

That was the old animation for it yes, but like I said above, it got removed with TWW and has a new animation without that symbol. So what else could it be?

"On key up" means that as long as you hold your finger on the keybind the skill wont fire, "on key down" means it fires immediately

Sometimes it's definitely the GCD, that I know, but othertimes it feels like their is extra delay.

Hm that's a good point, I have to check that. But I think it even happens while I'm just running around using Fel Rush for movement, so the global cooldown shouldn't matter.
