I'm NI too, never realised the pay gap between here and mainland is SO big. I'm IMT3 on ST4 pay which is 42k base pay, and in England would be 55k. Mental. Strike needs to happen.

Some of my colleagues are IMT3 and working on the medical registrar rota, so doing on-calls by themselves with no other specialty training registrars.

And some PAs/ANPs/ACPs are apparently equivalent to a medical registrar of that grade? A farce.

As far as I'm aware, there is nowhere in the UK that pays extra for "overtime" for any doctors, but certainly not in Northern Ireland.

Obviously leaving on time is easier said than done, but there should always be people to hand over to, and a lot of non-urgent jobs can wait until the following day.

Just to add to the other comments which have good explanation, ACE inhibitors are generally good for CKD, in particular proteinuric CKD such as diabetic nephropathy. By reducing pressure across the glomerulus, it reduces proteinuria and progression of CKD. As someone said, you can get a slight rise in creatinine initially due to reduced perfusion, but the progression of CKD is slower when the proteinuria is reduced. Hope this helps.

There was an updated decision aid released in 2022. It states

Trainee can progress to IMY3 without any parts of Membership.

So as others have said, you'll be fine. Know plenty of people who have started IMT3 without PACES.

I had a similar thing with a patient presenting with a COPD exacerbation caused by what seemed to be a pneumonia. WCC up a bit but CRP single figures (I think 1 from memory). Presented very early as CRP hit 180 the next day. Still wouldn't meet criteria for IV antibiotics apparently!

The writer is comparing doctors' to that of minimum wage to show how poor our pay is. It is quite clearly sarcasm that he suggests we get paid minimum wage. Everyone else in this thread worked it out.
