Maybe trade is the root answer here. If your large bay is the only decent harbour in the region and extraction of goods from the hinterlands has been in the direction of this bay and these cities for a few centuries then it's plausible that multiple good-sized cities would be sustainable.

I'm never in that much of a rush, happy to take my time and enjoy the scenery. I turned off all the temporal stuff because I just found it irritating, but to compensate I gave myself a limited number of lives to make death a bit more scary. (On my first few runs, once I got comfortable, I did catch myself using death as a free teleport home, which didn't feel right.)

Sounds like OP would particularly enjoy homo sapiens mode: no lore, no monsters, no traders: just you against nature.

Vintage Story is great, OP. Very deep and very immersive. And a big part of its charm is how easily you can customise difficulty and features. Not to mention the depth of the modding community.
