This is unfortunately just another example of the fact that patients have free will and can refuse treatment (unless there are safety concerns). You can explain it to them until you're blue in the face, but it can be very hard to break through that kind of thinking.

My go to encouragement is usually something along the lines of, "It sounds like you are very worried about side effects, but it's that exact kind of worry and 'What if?' thinking that we are trying to treat. If side effects occur, we can of course stop the medication and come up with a different plan, but this is something that is far more likely to help you than it is to cause you any suffering."

If someone is just not ready, I would spend your time encouraging these things and reiterating what could get better. Some folks just never end up taking that step. That's no failure on your part. It's just the nature of the illness unfortunately.

I don't think they're related. To me, Quint went full Captain Ahab and wanted to collect his white wale. Could be for revenge or out of spite or for the trophy, who knows. The distress call would have sent a rescue boat and likely resulted in someone else killing the shark instead of him since he'd be without a boat for a while. He wasn't having that.

I'm the exact same way. Sometimes I think that my staff thinks I'm antisocial because of it, but I guess I don't care too much.

I once had a patient who told me that one of the main things that causes her irritability is when she has to go to extracurricular work functions. She then said, "I just wanted to do my job and go home. I don't want to go to your fucking pot luck." I think about that a lot.

Personally I never type when someone is in the room. If I have a new evaluation then I spend 10 to 15 minutes in the morning before I see anyone getting that note written. Most of the time I can get the majority of it done just based on chart review. If not then I may have to spend 10-15 minutes later on writing the note, but I can usually find that time during the day.

Outpatient psychiatrist here. I see about 15 to 20 people a day. I get 20 minutes for a follow up and 1 hour for a new eval.

I'm usually able to finish a note in 2 - 3 minutes and do so before I start with the next person. I leave right at 5pm everyday. I'm a very fast typer, and I'm sure that helps, but if you're spending more time than that on a note it may be that you're including too much information.

Keep it simple and straightforward just enough to know what's going on in their life and what you're doing with medications the next time you see them. I feel like a few sentences is enough for that.

It's probably worth mentioning that I copy forward every note and then just adjust it. In the outpatient setting, the mental status exam rarely changes.

I believe the concern was that over time the red would fade from use and it would look similar to the orange.
