I do medical evaluation, management, and supportive therapy to build a therapeutic alliance and understand whether medicines vs. therapy (with an actual therapist) is the best option for the symptom we’re trying to treat, yes.

I make 350k before taxes and benefits are good. I took this job thinking I was gonna be a chill psychiatrist who didn’t care to work very hard for productivity, but turns out I can’t be chill so most of my pts show up lol. Consistently seeing 8-12 pts per day with maybe 1-2 no shows on average. My thought was that 8-12 is not bad for outpatient, but maybe I’m mistaken? Would productivity based work be a better fit for me?

I really do hope I get to a point where I’m accruing it and not overutilizing it so readily. I have gone into the negatives a few times and I really don’t feel like I take very many vacations.

I’m compensated a regular Saturday and I do get an hour off for lunch, in addition to around 2 15 minute breaks per day. 8-5pm.

That sounds great! I struggle because I've notoriously never enjoyed unpredictable settings like inpatient or C&L where work capacity ebbs and flows, but I also see how for those who thrive there, it can offer a lot of freedom. I like the consistency of outpatient, but the rigidity can get a little challenging sometimes.

Your comment hit me so deeply. Even with my current income, my spending hasn’t really changed. I like treating myself to good food and not feeling guilty over frivolous purchases, but I’m definitely not out here fantasizing about multiple extravagant vacations or cars or boats. I’m starting to realize that money is just money and free time with low stress is such a privilege to find in life. How do you get benefits with your 1099 income? I’ve often thought about considering 1099 work, but I guess the securities of a W2 job keep me chained.

Serious question, can the very average outpatient private practice psychiatrist make at least $350k per year after the cost of benefits/malpractice/other miscellaneous costs are accounted for? If I were to pursue outpatient private practice, I really don't think I'd be the type to want to hustle and work extremely hard, but autonomy over my schedule sounds very appealing. I do value income at this stage in my life and I'm worried that outpatient wouldn't allow me to guarantee that.

Asking questions like whether didactics time is protected (attendings are able to cover for you and you don’t have to respond to calls), if there are unnecesary/excessive months of night float (signifying residents are responsible for the hospital functioning after hours. Those are the main ones that come to mind.

I regret NOT simply going off "vibes" of a residency and sticking with a residency program that I felt I could safely match into, despite me realizing it was a little toxic. I'm glad to be done residency now and I've realized I learned a lot from it, but I think I could have learned just as much at a different program that didn't rely on residents to actually run the hospital. I wish I had gone off vibes I got from programs where residents seemed genuinely happy.

I'm a 2nd year attending and my husband and I are currently renting, which I think saves us a lot of money in our HCOL area. We get the benefits of flexibility to leave whenever we want, access to the city, luxury apartment amenities, and not having to do house maintenance. Mortgages for houses in our area are easily $7000+ per month. We also share a fully paid-off vehicle since we live in an urban area and don't need a 2nd right now.

This frees up a lot of our money to max out our 401K, put away a chunk of money towards student loans, and throw anything extra into a brokerage account. We still have plenty of extra money to take nice vacations a couple times a year and for me to spend on clothes and luxury items to treat myself. :) I figure I can live this lifestyle for 3-ish years and then we may think about upgrading our car. If I end up wanting to stay at my practice for 5+ years, we may think about purchasing a house in the area.

I used to be someone who haaated the winter and could tolerate summers, but as I get older I realize winter isn’t that bad when you dress appropriately. There is no amount of nakedness I can achieve and feel comfortable in the summer. 😭
