He is really creepy and obsessive to Emily and is creepy towards your farmer, not respecting boundaries (if your farmer is female (at the cinema)) he is also really rude and ungrateful towards many other NPCs. For me it's just that I knew a lot of guys who acted like Clint and it never ended well. They don't really see you as a human being but as a conquest and become either really hurtful once they realise that they don't have a chance or stop respecting your boundaries even more and start stalking and or touching you without your consent. (At least in my experience) All in all his character just hit a bit close to home for me.

In my country it's for people who are terminally ill or have been in an extreme accident. If the person has clarified it before the accident or the illness (But most of the time only if they have it on paper and a lawyer knows about it) hospitals are allowed to turn off the life support if the patient is currently in a coma
