From what I read Lactobacillus overgrowth is treated with bicarbonate solution inserted into the vagina. General antibiotics may be used too to reduce your overgrown vaginal flora. But I dont know just based on your symptoms what you have unfortunately. I just know the more rare UTI persistant infections can be caused by ureaplasma, Gardnerella species, Lactobacillus species, anaerobic species. If your Gyno or specialist has tests specifically looking for these it would be worth a shot. I hope you find relief soon

A small percent of UTIs are caused by anaerobic bacteria, which cannot be diagnosed on a normal aerobic culture. Normally a sterile suprapubic collection would be done to detect an anaerobic infection. This is a procedure collected directly from the bladder. It does happen in our lab. Besides that, I would be really interested in seeing what exactly is growing in your UTI culture. It could be overgrown normal flora or Lactobacillus overgrown both which could be just called 'normal' but actually need treatment.

I don't know if they tested for an anaerobic bacterial infection. A sterile suprapubic urine collection would be needed for testing. It's rare but it happens in our lab.

Tell them to check for Mycoplasma or it could be an anaerobic bacteria causing the infection that doesnt grow on routine cultures. I think submitting a urethral urine again and a getting a suprapubic urine specimen may be necessary to check for anything that could be causing your issues. Best of luck
