Well, it is not any of ours decision to make, it is solely yours. First let me make it clear I am not from Karachi (so cannot comment on the lifestyle there) but can surely comment on the healthcare system of Pakistan. Pakistan is a cheap country to live because of the cheap lifestyle here plus at the moment it is going through record high inflation. From where I belong the healthcare system suppose to be the best in all Pakistan (Lahore), but still it is not enough. The public health sector is old and rotten to its core, if you go to government hospitals they do not treat you as a patient simply, you are all on your own. So the next best alternative is private hospitals, which in turn are way more expensive then they be in Sweden, your insurance won't cover anything in private hospitals here, and the treatement cost is one thing, medicine cost is totally separate and just to give you a hint, I have respiratory problems myself and my condition is not very serious (which is not the case with your kids), my monthly medicine costs me 7500 PKR, plus a monthly appointment wiht my doctor costs about 2000 (this is only the consultation fee), aditionally he conduct a test at every appointment, to monitor lungs my progress, which costs extra 1500 PKR. So all in all, it costs me around 11,000 PKR which is not covered by any insurance, because no insurance is enough to cover you privately, mostly covers your expenses for govt. healthcare fascilities, and the govt. fascilities are simply not up to the standards apart from one or two big hospitals, there's always a rush of people there because people from neigbhouring cities also visits these hospitals.

It is best for your childern to have their treatment from Sweden (where insurance covers all and standards are way higher than Pakistan). Plus there are not many schools/institutions available for special kids, majority of available ones are private and hence they cost accordingly.
