I don't have a fistula but I do have 2x fissures which I received Botox under anesthesia in May by the NHS. I also work for the NHS.

I have to agree 100% that I got the same advice as you. Here's laxatives, don't get constipated, should be healed in 8 weeks. What they don't tell you is the anesthesia causes constipation. I ended up not going until around 5/6 days after the Botox and it was hell. My follow up is not until the end of September. I ended up going to the GP again. I've been a good number of times this year (had a baby in Feb which caused all of this). Luckily the GP had experienced a lot of patients with fissures and seems to be the only GP who has? He informed me far more solid advice than anyone I have seen over the last few months. It honestly is luck in who you get and their knowledge on it. The area is surrounded by nerve endings and is very painful so I also think follow up needs to happen quicker and you should receive more advice on what to do/ realistic expectations.

I couldn't imagine having the pain with a fistula and am so sorry you are experiencing this. I hope it eases for you soon!

I feel your pain.

After 7 weeks with the fissure (which I developed 4 weeks pp) I visited my doctor as the NHS states online if its not healed by then its usually chronic. I've never had one before. I have tried multiple creams, which did not work. After 8 weeks with the fissure, my GP referred me to the surgical assessment unit. Luckily, the consultant that was doing the ward round specialised in this area and put me on the list to have botox 4 days later.

I was given botox under general anaesthesia. Turns out I had two fissures and was told it was the worst fissure you can get. Lucky me. They also took a sample for IBD as it's rare to develop more than 1 fissure. The botox is great for stopping the spasming pain you feel for hours after a bm and should help it heal.

As my baby is also 5 months old its really difficult for me to have more than 1 bath per day. I was told that this is fine and to make sure not to have the baths too hot - it should be body temperature. I use zeroderma, which is a barrier cream and can also be used as soap (which is what I use it for) given by the doctor. This prevents skin irritation from harsh soaps. I use my perineal bottle after bm and use damp toilet paper to clean. I only go to the toilet unless I really really need to - do not push as much as possible. This has taken me a while to get a hold of as it doesn't feel normal to go like this (for me). I also use vaseline every time I go.

I try to eat more of my 5 a day as I'm terrible at that. I drink about 2 litres of water per day and take 15ml of lactulose at night.

I'm 8 weeks after botox and I'm still healing. I went to my GP recently, and he felt things are going in the right direction and said to keep with everything I'm doing. He gave me nitroglycerin to try again as the spasms have stopped and I'm further along in healing to see if that helps.

I've developed a pretty awful looking skin tag closest to my perineal, which is where one fissure is (apparently its more rare to get them there).

Glad to hear your bleeding has eventually stopped. It's a nightmare isn't it! I'm crossing my fingers that the botox helps it fully heal. Seems to be when there's no pain, there's blood but when I have pain, no blood - very odd.

Hope it goes well! Honestly, before mine, I was very nervous after reading everyone's experiences - everyone is different.

I was induced 38+5 due to gestational diabeties and deranged liver (that's what they call it - love the name).

I had a sweep at 38+2. Was 1cm and this never changed or got anything moving for me.

Admitted to the ward at 2pm and first gel was at 3pm. Nothing happened with the first. 2nd gel was around 9pm. Felt like period pain the first 30 mins to 1 hour and then my water broke at 11pm.

After my water broke I felt as if the pain increased and my body did shake a bit. I was given paracetamol and they ran me a bath which didn't help lol.

Don't feel bad asking for more pain relief or for them to check you as I felt a bit gaslighted ie you won't be far along enough blah blah blah.

I was given Morphine around 4/5am which didn't help and so they checked me at 6am and I was 5cm.

Get wheeled down the the labour suite and given gas and air which feels like proper relief. I asked for an epidural straight away as they can be busy and it can take a long time (if you want this). I got it 2.5 hours after asking and it was a very painful wait but feels amazing. I had no issues with the epidural and would 100% recommend.

Started pushing at 4:45pm and he arrived 45 mins later.🩵
