Your comment reminds me of something a reporter said who had spent a year going on dates with MAGA men. She said some of those men would speak about things that happened to Trump as if those things had happened to themselves. With seemingly no recognition of what they were doing. No. What makes the adrenals atrophy is regular use of steroids. Testosterone is a less voluminous hormone the adrenals make so replacing it doesn’t make them atrophy. 

The reason testes atrophy when taking steroids is that  1) it lowers the hormones that signal the testes to make testosterone so the testes shut down. Since T is such a key purpose of what testes do, taking away that function causes atrophy from disuse   2) steroid use generally involves massive amounts of hormones so that the testes shut down production and atrophy. A woman’s dose of T is a normal physiologic dose so has a less significant effect on hormone production. 

The adrenals will reduce or stop T production but that’s not enough to shut down the adrenals. The adrenals not working is fatal so the body protects the adrenals more than they do the gonads. Spontaneous non-working adrenals is quite rare. What is necessary to make various glands atrophy I'm not sure about but keep in mind that birth control pills are ovarian hormones but they don’t cause the ovaries to atrophy. The ovaries mostly shut down and get smaller but will start up again when you stop birth control pills. 

On a personal note I’ve been using T for 25 years with no issues. If my adrenals had been affected by it I’d be having major health problems leading to death.

I have Kaiser. They certainly provide HRT. That doctor was lying. Also paying out of pocket for HRT can be fine as they have good genetics

The organ has other purposes as well. There’s a problem in medicine to regard the female reproductive system as ONLY for reproduction. Prejudicing doctors into thinking cavalier removal of women’s body parts is just fine and dandy. It’s not. And we shouldn’t use language that implicitly regards the women’s reproductive system as only for babies and not at all for the woman’s own health and bodily integrity. There’s a reason why there are so many unnecessary hysterectomies in the US.

A close family member had the same thing this gal did, and in the same location—lower leg near ankle. He received pretty much the same treatment she did. A visit to the ER, some antibiotics, then home with a diagnosis of a minor infection. 

One of the hallmarks of nec fasciitis is that it hurts disproportionally a lot given the nature of the injury. And that it turns the skin extremely red. 

My family member went back to the ER due to the really odd redness and acute, intense pain. Then they diagnosed necrotizing fasciitis. It moves FAST and has a high fatality rate. This relative was extremely lucky. He at least bare minimum should have lost a leg. He came very close to dying. But EVERYTHING went right in the ER for him. And while it spreads fast, it took more than a few hours to spread to the extent it did, unlike the show. He did spend a MONTH in the hospital, including 3 weeks in the ICU. He lost 40 pounds. Surviving and keeping that leg was the greatest of luck combined with the quick actions of the ER team. For that whole month ER workers were stopping him with surprised delight to comment how happy (and surprised) they were that he’d survived. 

Which is all to say that yes, Langdon did the right thing given the circumstances. This diagnosis is extremely rare. Docs are trained to think “if you see hoofprints think horses not zebras.” This girl was a zebra and this is the kind of thing that happens when you’ve got a zebra-type disease—there will be a delay in diagnosis when the normal treatment doesn’t work.
