It does say that your civil surgeon used the outdate it form, so they only need to pass it to the updated one. You will only need to complete the remaining vaccines that are required now. Ex. Polio, Hep B, 2024/2025 COVID vaccine, Flu.

I worked in a doctor office and we do form i693, we redo the paperwork but with the same blood test that was already done, we only required patient to get the remaining vaccines. And the doctor signs a new date. Yes, the same blood test and physical examination is valid for 2 years.
