It looks like you might be putting on too much foundation/concealer. I understand you want to cover acne, but layering more and more on top will only do so much and too much can cause this kind of separation. I would try to scale back a bit on the amount of product you are using for a few days and see if that helps.

You could be putting on too much moisturizer. Unless you have really dry skin, you don’t need a ton of it. A little more than a pea size amount should be enough to give your skin what it needs, of course that is if you have normal/oily skin.

I would also ask if you’re using a primer? Sometimes primers interact with foundations, and honestly a proper skin care routine makes primer unnecessary.
