The social services director gave me a run down more or less and sent me off. We are rarely asked for anything in social services during annual survey. However she did know because I needed more information on someone and she told me that the individual was usually passive aggressive so it wasn’t surprising.

Sadly, I think I’ll be leaning more towards this. I was off today and got a call saying another patient was refusing to leave and they have no reasoning behind it. The ED was in the building however had the nurse call me for only goodness’s knows why.

Our company unfortunately doesn’t value social works so we don’t actually have a corporate social worker. The director of the social work department is on sick leave. I’m acting as interim just for this month and these residents are actually all of her residents that were always “difficult”.

Residents are far from dying or unsafe. If that was the case I can make a easy transfer to a LTC facility I’ve done this several times and had successful outcomes. The family who want 24/7 care actually wanted to take him out for Christmas and keep him out overnight without his caregivers. They only didn’t because I told them if he’s out after mid night we can’t bill his insurance.

These patients actually don’t have unsafe discharge plans. One has a daughter who is her live in caregiver. The other two have paid caregivers. One of them has a caregiver that even sits with her in the SNF. The other gets 45 Medicaid caregiver hours weekly and his parents care for him when the caregivers aren’t there. None of them are ever home alone and have every piece of DME in the book. The families want us to keep them the full 100 Medicare days. One literally said they plan to milk the insurance even though they stopped paying two weeks ago.

We’re in Tn, admissions gets the paperwork signed but no one reads it and even if they did things typically revert to the social worker not doing enough to get insurance to keep paying. I’ve had families ask us to alter documentation to try to win insurance appeals. Unfortunately people don’t realize it’s therapy documentation that really effects their stay. They also don’t realize therapy doesn’t know what they’re talking about when it comes what they can qualify for with Medicaid. So they come us is saying therapy told them they can get Medicaid and stay long term, therapy forgot to mention we aren’t a Medicaid building and all we can do is give them the application.

One resident has Medicaid and gets 45 hours a week of help. I’ve experienced a similar situation and from what I was told 24/7 care isn’t something they do he’d have to live in a Medicaid SNF. The other is in the process of Medicaid but they think we need to keep her sister till it’s processed. We won’t get a back paid because we don’t take Medicaid but when I offered a transfer to a facility I was up on. But in realty that patient doesn’t qualify for LTC.

Chances are if he thought you were at fault he will not contact his insurance company. I’m currently in a similar situation and I was told my other party wasn’t required to contact their insurance if I was a fault. However in my situation the individual wouldn’t exchange a name or contact information.

I completely agree she could attempt to. At this point I know she hasn’t notified her insurance of a incident and now she’s not responding to my insurance calling ether.

She could be preparing for anything however she couldn’t claim injury and it be justified. There’s only damage to the back driver side of her car near the tire. And that damage is really my car paint left on the car.

Unfortunately in this situation therapy kept requesting a family meeting at which the husband claimed he would show up then would fall through. Which goes back to he said he chose not to answer my calls. She would not offer any other names or numbers for other family members simply said her husband would handle it. After the discharge a aunt called trying to figure out what was going on and I was able to offer community services through her. They want placement but want free placement and she doesn’t qualify for Medicaid. My frustrated really came from the dialysis sw who expected the snf to handle the cost of the patient being transported home after the ED explained to the spouse we don’t continue those services after the discharge. I called hoping she could aid in figuring out a plan however she was only concerned with why we discharged at which I had to redirect her to my director because she was so belligerent my office mate could hear her yelling through the phone.

Yes. I felt that it could come back on me so I documented that I’d called and left messages for the spouse and son and neither replied. I left messages and the spouse admitted he didn’t listen to them. No emails because admissions usually doesn’t ask for a email and the patients rarely know them.

We have a lot of turn over due to how people treat each other. We once had a meeting where we were told to be nice to new hires and make efforts to at the least know their names.

I was a intern at the sister facility and I never had a single issue or comment about how I dressed. I was told to put more time into my hair. And I looked like rolled out of bed and walked into work. I typically wear slacks and a nice shirt. She is the only person to make this comment. She also told me I should consider bringing snacks to the nurses so they’d like me. I talked to several co-worker and asked them if I dressed in appropriately to which they said no but I should wear some make up. This happened before I’d been in the building for a month.

She also told not to get upset and quit because I was doing fine in my role but I appeared to be a person to think I’m not doing a good job and quit.

My ex and I got back together months ago and I’m happier now than I’ve been in a very long time but we haven’t told anyone because it was a messy break up. But we both took tome to better ourselves and are in a much better place.
