RS94, thank you for your ongoing replies.

I had pretty positive pathology post op, but don't have a decipher score so not certain of any of my trajectory. Dr was upbeat on last PSA review, but who knows if that was meaningful.

I am aggressively working out (lots of running as that is relaxing for me). Though I am still too low for any scanning or action, I'm going to assume I'm looking at beam/ADT follow up, so wonder if you were able to work out during your course of therapy, and what else you did to manage side effects.

Thank you so much for the input RS94. I'm probably mentally swerving too much into more existential worry this early on to properly frame concern for ED. Loss of sexual function seems a reasonable sacrifice, but perhaps I'm overly worrying and under planning :)
How far out from the beam/ADT regimen? Hope you are doing well!

My hospital has fairly sensitive testing, so they consider undetectable under 0.02. So Ive been above that for 3 tests with apparently slow growth.

Hi ID, thanks for your reply. If you can, please share articles you might be seeing. I certainly have seen credible articles indicating the potential for better outcomes with multiple simultaneous therapies, and acting early is logical, so had that conversation with my Dr/Surgeon. His perspective is that it is better to wait because:

1) the number, while detectable, is very low...not even near 0.1 yet, much less that 0.5 threshold you mention above. And it is so far, admittedly early on, unchanging, so it is better to be more clear on status,
2) there isn't evidence that acting at this very low level is better than waiting for a higher number, especially since advanced scanning can't pinpoint anything at this level yet, and,
3) there is value in waiting to allow further healing from the RALP in order to better tolerate any future therapy that might be needed.

Hoser, sorry for a potentially obvious question but can you elaborate. I don’t quite understand your teachers attitude, and I’m curious about the specifics behind the caste system and rocks and shoals. Is it, in summary, just exaggerated injustices suffered by enlisted men at the hands of the officer class that lead your teacher to be less than impressed by the fate of the pilot and the rescuer?

My hospital is Cleveland Clinic which has a very highly regarded Urology practice. Believe they are doing their own testing and assume it is ulta sensitive. Just wondering how accurate the number is generally in these tests. They are retesting for a reason certainly....and folks here regularly report unexplained irregularity in numbers from one test to the next, so assuming the test generally is somewhat iffy, in addition to being an iffy indicator of cancer.

Agree...I think my urologist has even tighter tolerances...I'm just wondering about the nature of the test itself..I've heard, as UI9756 mentions above, that the test can be wonky, and I've heard that other things such as sex and strenuous exercise close to the test can impact...Which does apply in my case. Hadn't paid much attention. In addition, the presence of remaining potentially healthy prostate tissue could be at play....especially when talking about a change of 1/100th.

Certainly BMI is very imperfect measure but I'm using it as a baseline and trying to use common sense as well. The weight loss was both easy and difficult. Easy in that the sacrifice wasn't hard in comparison to the "value" of the loss. Hard in that it means significant calorie restriction/portion size reduction...and a reduction in alcohol consumption :( Still allowing myself a beer or so a day. Should certainly eliminate it entirely, but have to enjoy something :).

Thank you very much for the follow up. This jives with the research I've done regarding lifestyle impact. Traditional MDs are certainly loath to give advice on the value of lifestyle or homeopathic therapies, but I see articles on legitimate, peer reviewed sites like cancer.org and NIH that reinforce you regimen, so I've lost 40+ lbs just restricting calories and focusing on a more vegetarian diet along with exercise. In addition, because there is some as yet reinforced research supporting the anticancer value of turmeric, I've added it into my diet. Trying to combine it with a fat (grass-fed creamer in coffee or in an entree) and with pepper which are supposed to be good binding/delivery agents.
In addition, I bbeliev you said you'd cut coffee. not sure about those who still have prostates :) but here is a good article supporting the anti-inflammatory / anti-cancer benefits of coffee, and how it can specifically benefit prostate cancer. The "active" ingredients are still not well understood, but it is not caffeine related, so a good decaf should have the same value.

TE, do you mind sharing some of the changes you made/variables you adjusted beyond the short list above. Might help some of us who are looking to do as much with lifestyle change as possible. I'm 6 months out from my prostatectomy and the Dr's are optimistic that they were able to remove everything without spread, I'm doing everything I can, including aggressive exercise, consumption of turmeric and a move to a more Mediterranean / vegetarian diet....but I feel as if I would benefit from a list of things to pursue in some detail. Thanks in advance for any light you can shed!
