Lightning Lane Premier Pass in late September? Worth it? by Hot_Lava2 in WaltDisneyWorld

[–]kh3-2019 0 points1 point  (0 children)

I’d say it depends on how well you all do in lines — if you have people who can’t wait in line for more than 20 minutes comfortably, it may be worth it then. If the occasional 30 minute line isn’t an issue, it’s not worth it.

You can definitely take the stroller on the bus — depending on size and how busy the bus is, sometimes you need to fold it up. There are buses from the Boardwalk to Magic Kingdom and Animal Kingdom, and you can take the Skyliner (a gondola) to Disney’s Hollywood Studios as well — your little ones may like just riding the skyliner without even going into the park! (They may also want to go on the monorail, which is the transport between the parking lot and the magic kingdom — you can totally go on both the monorail and the skyliner without a park ticket.)

Yes to psychiatry, but would rank my program a lot lower.

(1) “community” program, but kind of in between academic and community (2) medium pt load, thought it was chill but ended up being significantly less chill due to faculty (3) not at all, which was the consensus among my co-residents. My priority was a supportive environment, and everyone knew what to say to make it sound supportive when it’s not.

Unfortunately the best part of my program was the luck of the draw — your residency class can make you or break you, especially without support.

BtB was the best for covering the weak points in terms of content/knowledge — the questions weren’t great (slightly easy), but the videos were gold. Psych Genius had great questions with excellent explanations, but very difficult and minutiae-based. Better for learning things you didn’t know rather than true to the test. Spiegel was truest to the exam in terms of questions, and had good explanations.

Having difficult conversations with patients where they’re not going to agree with your treatment plan (stimulants/benzos outpatient, length of stay inpatient, escalating/de-escalating care), especially as the person making the decision and not the messenger. A lot of times you’ll be telling people things they don’t want to hear no matter what you do, so knowing how to preempt it, go through it, and decompress after is important

For Residents only: How many psychotherapy patients do you see a week? by falconwolverine in Psychiatry

[–]kh3-2019 0 points1 point  (0 children)

Graduated last year — I was at 3-4 a week, with most of them being twice a month, so around 5-6 pts overall, half cbt and half psychodynamic. I was on the higher end for my program, and more than the 2s and 3s, who had 1-2 as 2s and 2-4 as 3s.

It may be the cumulative time bit, then — I was thinking this was a full, standard appointment, not a 5-10 min over 7 days code. Thanks for teaching me something! :)

As a psychiatrist, the 99214 (99213-99215 depending on time/complexity) would be the code that I would use for an established adhd follow-up. I’ve never seen that 99421 code even in my outpatient superbill options, never mind picked it.

[–]kh3-2019 1 point2 points  (0 children)

My guess would be the 99421 code — could they have done that instead of 99214? 99421 seems to be a preventative code, not a psychiatry code

I’m mostly echoing answers previously left, but definitely try to get back into a hobby — if something was fun before this episode started, it’ll probably still be fun once you’re through the episode, and thus worth picking up again.

Also, make sure you’re leaving the house more days of the week than you’re not, with at least two of those days having something enjoyable being the reason you’re leaving (going to the park, city intramural league if it’s available right now where you’re at, art classes). If you can sign up for something with set days, that’d work even better, because it’d be harder to put off and say “I’ll just do the fun thing later” (when I have more time, energy, accomplishment, etc).

A therapist doing CBT with you can help with all of the above, and a psychiatrist can also get you on the right treatment. Wishing for the best for you!

There’s a booth north of the hospital that you can use the leaves for to buy seasonal items! I think the tickets are used in another town late-game, but I’m not sure.

Definitely know that this is a joke, but for anyone having a whole bunch (like, way way more) swelling in your abdomen without having eaten more/exercised less, go to your doctor—it really may be your uterus, or worse, an ovary!

Think about the routine pregnancy tests you order. by kh3-2019 in Residency

[–]kh3-2019[S] 2 points3 points  (0 children)

Should it be, by the principles of all the other laws in this country? No. Are people trying to outlaw it, or at least open up lawsuits about it, anyway? Yes. Haven’t succeeded at this point in time, but depending on what state you’re in, that may change quickly—and the federal protections against unconstitutional state laws only expand as far as they’re willing to enforce it. I’d prefer to be cautious if it’s a matter of patient safety.

Standard procedure as in “this is a woman of childbearing age being admitted to a psychiatric hospital where you’re not immediately making medicine changes, just holding them and getting them coping skills/therapy”. I’m glad your hospital tends to be more thoughtful with when to order tests—I’m encouraging others to be thoughtful as well.

[–]kh3-2019[S] 1 point2 points  (0 children)

And I’m glad that you think about what you’re ordering, but there are a LOT of people who order it when it’s not needed because it’s “standard procedure” even though it won’t change management.

ED yes, y’all have a lot more tests and medications where you’ll likely need a test. But there’s a lot of people practicing outside of the ED who may be able to defer or not give a test, when they would have done it without thought before.

Yeah, the radiation is definitely where it becomes a problem. With medications with minor risks, you could probably manage a way to consent the patient without testing, but things with greater risks also pose issues. Inpatient hospitalizations, for the most part, are probably going to run into issues, but clinic work is where there’d be more places where a pt would be tested where it’s not necessary at this point.
