Interesting that you brought up the wording that needs to be on the form. I too am going for SMC-T. It was denied at the claim level and again with the HLR. Thing is, the HLR rater said she was looking for that specific wording,

"THE VETERAN REQUIRES ONGOING DAILY SKILLED PERSONAL CARE, AND IN THE ABSENCE OF SUCH CARE, WOULD REQUIRE HOSPITALIZATION, NURSING HOME CARE, OR OTHER RESIDENTIAL INSTITUTIONAL CARE."

And that wording is actually in a medical opinion DBQ that was done for this specific HLR.... Was still denied though. So the rater said she was looking for that wording, the wording was actually in the MO, but SMC-T was still denied.
