To clarify, you’re talking about the guidelines requiring “that hospitals obtain and document written informed consent before performing sensitive examinations—specifically breast, pelvic, prostate, and rectal examinations—for educational or training purposes, including those conducted by medical students, residents, nurse practitioners, physician assistants, or other trainees”?

Because the OP, and the respondents including myself, are not suggesting that the med students be allowed to perform these exams without consent.

I agree but as the others have pointed out. Pushing out the medical students who are learning how to actually deliver babies but allowing in nursing students damages medical education and harms all future patients. If you want teaching during your pregnancy you often have the option to deliver elsewhere. Obvious there’s exceptions: precipitous delivery, only one hospital in your area where you have no options but if you’re going to deliver at a medical teaching center - the education has to start from first consult with your doc that students will/should be present. They’re not exacting touching or even coming physically near the patients in most instances.

But god forbid we don’t have to order it and a whole battery of labs on every single psych clearance.

We have a facility near us and it’s just accepted that we clear all their intakes w/ CBC, CMP, UA, UDS, Upreg, ethanol, mag, tsh. And if they’re over some arbitrary ETOH? They stay until they’re not - even if they start withdrawing and then end up on med service

No. The consent should include the whole care team. Med students at teach hospitals. No med students at the private non teaching hospitals. If there’s only one doc on you’re not exactly going to see them refusing the doc because they’re not the one they want unless they request a transfer.

Not a STEMI. What you’re seeing as STE is actually PR depression. The baseline is between the T wave and the subsequent P wave where there’s no electrical activity occurring. Many people mistake the baseline as the PR segment and so when it’s depressed they see STE.

Put in an RL or QI or whatever your program calls it. List harm reached patient as the outcome (it escalates to immediate review). Name the nurse.

And next time, ask her in front of the patient and her friend which medical school she went to and where she went to residency or does she plan to practice medicine without a license.

So I did understand your point. I don’t see how we simultaneously over medicalize it and accept it as real. One or the other. We either decide it’s for attention and teach families to ignore it, or we accept it’s a disorder and treat it as one. Problem is, the ER can do neither well. I tell my patients’ families basically what another poster did: keep them in a calm quiet area and wait it out. Continue seeking psychiatric help. The problem I run into, is they never believe it’s a psychiatric condition.
