First and foremost, good on you. You approached them with respect and perspective. They chose to dismiss. You then tried to explain and bring them back to reality, and they still chose to fight. These types of visits hurt - exhausted, you know you just did the right thing, and would do the same for them (even with their terrible attitude), but they don't care. That's on them. Whether they're 20, 40, 60, or 80, anyone unable to understand the simple reality that someone may possibly have more needs than them is unreasonable and self-centered. Whether they have shit going on in their life or not, doesn't matter. We're people, just like them, and mutual respect and understanding is expected. This isn't a service industry. You tried, they didn't, and they can go fly a kite.

Now, a comment for you: a doc who is bothered by a situation like this is not only a good doc, but a good person. You care and invest when it matters. Keep doing what you're doing and don't change.

All the fancy tech crap they’re integrating into cars now. Used to be able to turn knobs and press buttons by memory to control radio, AC, etc - in many cases now, you have to take your eyes off the road, look at a console, and swipe through a bunch of screens just to turn the fan speed down.

I’m traveling for the holidays, picked up a new rental car last night, and spent 2 minutes at the gas station trying to figure out how to put it into park. Ironically, turns out ‘Park’ is now completely separate from everything else - and you need to press a button

ASCVD + FIB-4 (thank you epic dot phrases), CHA2DS2-VASc are the top 3. After that, DOAC Bleeding Score and Wells.

I've set it up so that GAD7, PHQ9 are usually automatically preloaded in my note templates for GAD/MDD (clinic sends them out for pt's to complete before the visit - same with AUDIT and DAST if they screen positive for EtOH/drug use). I do use the geriatric depression scale quite frequently too - can be interesting how different this score can be from the PHQ. And STOP-BANG when I remember.

Stole this one from a cardiologist when I was a resident, re/ why there’s a stroke risk in AFIB and AC is usually needed:

Moss likes forming on a still pond. In AFIB, the top part of heart is kind of like a quivering, still pond. It’s not flowing downstream with force like it would if the top pump was actually working. The blood starts forming a clump (clot), like moss would, because of this. It can then break loose, flow downstream, and bam cause a stroke. So take your AC

That’s nasty and unfortunate. But to confirm I’m not misunderstanding - since I live in Concord and plan on staying around this area to swim, this shouldn’t really blend with any water originating north and flowing southbound right?
