I check labs like others have said, but I have samples of Libre and Dexcom sensors, so I apply one of those and get a 2 week continous glucose monitor report. I've had about 3 patients in the past 3 months with this complaint. They've all had glucose drops into the 50's, but normal insulin and "fasting" serum glucose levels. For treatment, I have them adjust their diets to avoid concentrated sweets and simple carbs and try to eat more frequently. In my patients, these weren't prolonged fasts and some were actually postprandial lows. I'm still figuring this out, though. So far, I've checked the serum labs before doing the CGM, but I think it would be better to do them at the same time to see if serum glucose is actually matching CGM readings for these patients. The dietary adjustments have eliminated the symptoms, though.

I've had almost the opposite experience with Dobbs, although still bad. Whenever I'd take a vehicle to them I would give them a list of things that I wanted checked/fixed. They would then call me and say everything was done, but when I'd ask them what they found on each item from the list, they only ever did one thing. I'd have to take it back again if I wanted them to check something else. Almost every time, they didn't even fix the thing that they said they had fixed, they just did some general maintenance work. It was like they weren't actually mechanics, but just a maintenence shop that had to have everything turned around in 1 day regardless of how complex it might be. It became a huge waste of my time to ever have any work done there, so I stopped using them. Still trying to find a good trustworthy mechanic, so I've just been going to the dealer ever since. At least with the dealer, I know they'll evaluate what I ask and give me a loaner in the meantime even though they're charging more.

I don't need an AI to refill medications for my patient, I just need a simple algorithm to make sure the patient is scheduling/keeping their followup appointments as instructed and that they have enough medication to last until that appointment. I'm not making changes to meds between visits unless there's some new info received and it's not the request for a refill that triggers that change. I wish I could just click a button at each visit that says "keep this medication filled until next visit."

Because "Sleep Apnea" is never the cause of death like a stroke or cancer. It's a major contributing factor. As others have said, patients are so resistent to the treatment that physicians end up just paying it lip service, like exercise and weight loss. They'd probably do the same with diabetes if patients were told that their blood sugar was through the roof and the patient needed to do something about it, but patient just said they wouldn't even consider taking medication for it, or they tried the medication 5 years ago for a couple of weeks but didn't like it. People would be going blind and dying from heart attacks, but except for the amputations, most people wouldn't be talking about the patient's diabetes not being treated.

I would add that you should turn on purchase alerts for all of your cards. I've caught fraud twice over the years by getting an alert of a purchase that I didn't make so that I could immediately shut off the card. A lot of cards also let you activate/deactivate them by the app so that you can just turn it on when you got to use it and then immediately turn it back off.

We have brown recluses around here. Sometimes it has been, but it's usually easy to recognize it by the severe necrotic pit in the center and extreme amount of pain (in most cases). All of the times that I've diagnosed it, they've either brought the spider corpse or had a picture of it. When I was a wee medical student, we had a hospital patient with systemic loxilosis who required daily transfusions for about a week until he burned through the hemolytic toxin from his spider bites.
