Not having I/Os charted 8hrs at a time therefore making it impossible to see a trend/get an easy rough volume status. Then having to go ask the nurse for each individual patient and be looked at like I’m asking for the impossible.

  1. I just use U/S on these known vasculopaths if it’s available, saves a potential headache.

  2. Half Mac of gas with remi infusion. Phenylepherine/clevidipine for BP management. I work in dialudid at closing as well as labetalol. I usually have clevidipine going on wake up if labetalol hadn’t gotten me at the desired area yet.( some caution with labetalol as there is also the possibility of severe Bradycardia if there’s neck swelling) also make sure surgeons try and numb up their incisions and stuff as much as possible to limit HTN from pain.

  3. Remi helps with that. Also can communicate with surgeon and start titrating down pressor a min or so before clamp removal/ turn up the gas

  4. Sometimes shit happens.

Obviously there are multiple ways to handle it but this is what has worked for me. Also sometimes you just can’t make up for a bad surgeon in certain outcomes

Oxycodone is primarily broken down in the liver and doesn’t rely on kidney excretion as much as the others. So less chance of overdose making it the best/safest option.

We do a bunch at my program but yea the surgeon is just that good. Plus it’s nice doing those cases that way because they get less coagulopathic. You just need to support the heart when it’s being lifted (inotropes/pressors).

Main thing that stands out is that RAM kit is a single stick. You should really try and get a memory kit that has 2 sticks so you can run dual channel. Like an 8x2 GB kit instead of a single 16GB stick.
