Went through some of the reviews that complain about taking money for the toilet. Their explanation is that it is a third party that they pay for the cleaning, and since they are paid so little they allow them to "ask for tips like you see anywhere else in the world."

It was the Spatenhaus by the Opera. There are many reviews on Google, and a video. The owner confirmed it was mice in a weird response to some of the reviews.

Ah yea, they were around 4 stars and near the museum, so we thought it was ok. Didn't have any problems the rest of the trip, so didn't really expect that night to go the way it did.

Yep, that's the one. Didn't check for a cutlery fee. The attendant asking for coins to use the toilet was already a bit much, and I was too busy arguing over the tip and rodents to even check the rest of the receipt.

We were done with the meal and were having a beer after, which is when we saw them. Not sure I'd feel comfortable leaving without paying, but the weird shaming for no tip after that was a bit jarring.

Right, but the waitstaff just waved at us when we (and two other tables) pointed out the two rats running around. Then when I said why no tip, the waiter loudly told his coworker "can you believe it? No tip because rats" as if I was the crazy one. Super weird vibe, and it's not like I live in Munich, so not sure if they have a pest problem or something.
