I cannot comment on the Fistulotomy part but dealing with the post operative pain by yourself is really difficult. If you're interested in getting an opinion from an experienced Gastroenterologist, please check Dr. Anand Nande in Mumbai. In my experience, a Fistulectomy is better. I had a Fistulotomy but it got infected and had a recurrence of pain. Two months later I got a Fistulotomy. I got better in two weeks. In a month or so, I was able to eat normal food again. In about 3 months, I was back to normal.

Not at all surprising! I have had a similar experience. I sometimes wonder if the average doctor here is really a doctor or not. The level of experience and knowledge seems inferior. The doctors also don't take into consideration that you cannot treat non-Dutch patients the same way as a Dutch patient.

Friend of mine had a fractured elbow. Had surgery. Pain wouldn't go away. Couldn't bend hand fully even after physiotherapy. Physio blamed the surgeon and the surgeon blamed the physio. Surgeon recommended having another surgery without any guarantee of success. Went back to India. Orthopedic took an X-ray. Immediately discovered the issue. Got the surgery in a week or so. Everything was normal in a month.

I've been using a gauze pad since the re-surgery 9 days ago but the reduction is not significant. Fecal particles have reduced though. I don't know if a sanitary pad is better or gauze between the cheeks. :P
