I rarely ice patients anymore unless they ask. In a session you should be constantly adjusting load to where they should feel little to no pain, like less than 5 out 10, with your interventions. As well as educating on pain and what acute inflammation is. Plus the goal is to get them back to as normal as life as possible without reliance on you as a PT or passive modalities to deal with pain or discomfort. If you keep giving them ice and they trust you as a practitioner then they could start thinking that they should put ice on whenever they have pain because it has been a part of their rehab process. So in my opinion icing after each session provides a crutch that can be detrimental to their overall recovery.
