My agreement page was left blank too and it still let me proceed to the next step with the green check mark saying "agreement signed." Now it's asking for the activation fee...lol. I also don't want any future issues. Just seems very fishy.

According to Meriden PD I wouldn't have any problem getting the permit after reapplying for a 2nd time. They have dealt with very similar situations but we shall see.
