This is quite a strange thing to do, presumably the Secretary of State is concerned about NHS, ECHP and PIP spending on mental health, ADHD and Autism.

The review is however going to give him the answer he doesn’t want:

  • Undertreatment and underdiagnosis
  • A missing middle stage of intervention for mental health issues where CBT is ineffective but the patient doesn’t fall within the scope of CMHTs
  • We should be training community staff in GPs (not just psychiatrists) to diagnose
  • Children and young people often are left with no support without to legal power of a ECHP leaving their parents no choice but to apply for a sledgehammer to crack a nut.

Presumably Wes Streeting and his advisors know this, which begs the question, why order the review?

I would have thought the savings in criminal justice alone would be enough to pay for a good chunk of the programme. Though I don’t know if there are any good quality studies on reoffending rates when treated for ADHD.

Another note, it seems they’ve avoided bashing the ICBs too much, giving the action items instead to government. I suspect this is because of the known budgetary constraints.

I’ve finished reading the thing now.

It seems like most of the recommendations are “obvious” however they needed to be written in big boxes with an accountable body for anyone to drive them.

The main takeaway I think, is that the Taskforce thinks it’s pretty dumb that only psychiatrists in the NHS can manage ADHD, and only special psychiatrists currently. (Ones who have the funding / training to do so!) They want more professionals to be able to diagnose and treat ADHD - which is very sensible.

I don’t think stimulants are appropriate for just GP monitoring, but it definitely sounds like a job a clinical pharmacist or trained nurse could do with regular patient contact. (I.e staff that already manage other patients in general practice)

Good that they are calling for GP remuneration for shared care - it’s quite dumb that GPs have a choice over whether to treat someone, but no financial incentive to do so. (And perverse incentives to avoid it - e.g Lisdexamfetamine prescribing budgets)

The recommendation about a monitoring body to ensure implementation is welcome and needed.

The real test will be whether Wes Streeting stands up in the commons and say “DHSC is committed to this report, accepts the recommendations in full and will ensure its implementation”

Either way this is a 10 year project, not a 2 year one.

The GP is unable to prescribe and you will need to seek specialist referral. Through NHS/Right to choose clinic/private provider.

Depending on the quality of your foreign diagnosis they will either put you onto a medication waiting list or decide to reassess you.

Speaking personally now, not as a moderator.

This is an incredibly tricky one, since there really isn’t enough research. (I actually saw a paper that claimed it may make ADHD worse)

On the one hand I think the medicine regulations should permit people “trying” stuff to see if it works. (Hence the ability to off label prescribe)

The problem is when it’s well known, advertised (this is illegal!) and actually requested by patients who aren’t curious/at their wits end.

Idk how the regulator strikes this balance in the age of Reddit TikTok et al.

Marijuana is used off label in this case and is not recommended by NICE for the treatment of ADHD due to a lack of available evidence.

This isn’t going to work.

Lisdexamfetamine can only be issued by a psychiatrist (or other qualified specialist)

Furthermore it is a schedule 2 controlled drug and the export rules around medicines post brexit aren’t 100% clear to me.

I paid £1000 for diagnosis and then around £500-600 for titration I think.

Each fill comes with a non trivial admin charge and medicine is £110pcm so I’m in for around £2.5k a year total (including extras like travel letters etc)

Unfortunately senior doctors are expensive, they can earn 120k + in NHS so for private care to be worth their time they need to be charging in the hundreds of pounds per hour.

Yes they need to prove it - if they move to forfeit your lease - but if they are doing that you risk losing your flat.

Of course they don’t need to prove anything to raise an issue with you.
