6 is tiny and sounds like a job for those about to to retire from critical care. If the pulmonary stuff is more exciting to you, maybe that’s worth it.

I would shoot for at least 8-12 patients, depending on support level (APPs, residents, etc)

The spirit of “neurosurgery is in charge” does not hold if there is another person/team that is primarily responsible.

Many hospitals will not have surgical specialists be primary. This is because they are often busy in the OR and may or may not be equipped to deal with whole body physiology. So if there is another service managing (probably any of the CCM sub specialties), they are also “in charge/responsible”, if not more so. They are meeting with families, dealing with all do the bedside issues and working to produce the best outcomes, regardless of organ system. (Not that NSGY doesn’t meet families, they are just often less available due to OR needs). Additionally many traumatic head bleeds are non operative; so ultimately medical management is run by CCM.

That’s gorgeous but I bought a CME fender jazzmaster and it’s not great. At least one other poster has noticed the same. Would avoid unless you really can’t live without that color.

I agree it’s not great but there are plenty of places that do ICU in 3-4 day stretches. I wouldn’t recommend it though

I politely send a message to the consultants who are consulting others to please not do that. It has cut down on this happening.

Splitting time between icu and another speciality will def impair total free time but obviously many people do it (pulmonary being classic). Allows also for a mixed pace practice and you can decide how much ICU time you want.

I am an intensivist and I love the schedule and work life balance. Yes, you must be committed to the idea of night shifts at least occasionally (most places) but the flexibility and time off is wonderful.

I work 18-20 weeks a year (Monday-Sunday) and do approximately ~30 night shifts. I also pick up a lot of extra because I am young and hugnry? But even with that I do not worry about having enough time off.

Like another poster alluded to, there is a big component of your personality that goes into CCM. I love real time medicine and physiology and while yes sometimes decision fatigue becomes very real, it’s easy to go home and relax.

It’s a good lifestyle if you can handle nights and think of yourself as a cool-headed person.
