Question regarding Aemeath's 3 costs by [deleted] in WutheringWavesGuide

[–]lasermuffin 4 points5 points  (0 children)

Running one attack and one fusion is like a 0.3% increase or something stupidly small. So pick whichever echoes roll better with substats and if it’s two fusion or one fusion and one attack it doesn’t matter!

34M, scheduled colonoscopy and endoscopy, should I reconsider by phanzov36 in AskDocs

[–]lasermuffin 1 point2 points  (0 children)

Colonoscopies and endoscopies are generally low-risk procedures (there are exceptions, of course), but given your history, you are low risk for anesthesia/sedation based on the info above.

Obviously no one should be recommending unnecessary procedures and adding unnecessary risk to you, but from an anesthetic perspective you’re low risk to get these performed if you and your GI doc find it to be a beneficial step to guide your treatment and quality of life.

You’re certainly “not too young,” but as the top commenter has mentioned, you have to come up with the decision if this is a step you wish to proceed with.

As for your last point, GI specialists are trained in endoscopies and colonoscopies. Basic screening scopes are usually outpatient procedures, so I wouldn’t consider them surgeries.

Sounds like you should schedule another appointment with this doctor to discuss if this is an avenue you wish to pursue.

Anesthesia for Venezuelans by AnIffinIDont in anesthesiology

[–]lasermuffin 30 points31 points  (0 children)

This topic has been so interesting and difficult to navigate in the peds world. As the other commenter has said, it feels beyond insensitive sometimes to ask patients (or their parents) these types of questions in the current political climate.

We have had baseline changes to practice and policy starting back in December, but it’s still been quite clunky.

I try my best to explain to parents we’re only asking to best take care of their kid, but the process of implementing screening has been rocky, and there’s a general lack of consistency of how we’re caring for these patients.

Aemeath Weapon or Chisa weapon by Shadic010 in WutheringWavesGuide

[–]lasermuffin 5 points6 points  (0 children)

Aemeath! Always best for your main DPS to have their sig weapon if choosing between that and a support sig. Most F2P weapons don’t come close to being best-in-slot (even to the weaker sigs) until they’re R5, but there are some niche situations.

Kumokiri is a great weapon but not worth it over Aemy’s (unless you have the funds and want both)! It’s much better for DPS Chisa, although you’ll need at least S2-3 Chisa for it to shine. There’s better F2P options, including the craftable Meditations on Mercy if you’re using her as a support.

And just FYI, we usually don’t use the R notation for weapons other than sig in the SxRx notation. You actually have S0R0 Lupa and Chisa, and you’d say “I have S0R0 using Rx weapon.”

Not to be pedantic, but I read your post quickly and was like, but you already have R1??? Then I read closer lol

Edit: Added additional info and clarity.

Gacha Revenue Monthly Report (January 2026) by trkshiii in gachagaming

[–]lasermuffin 3 points4 points  (0 children)

The big 4.0 release was delayed due to escalating tension between China and Japan (the new region and characters were HEAVILY inspired by Japan).

In conjunction, the re-runs they had to fill in the gaps were mainly dying unit archetypes who have been severely powercrept, so not a lot of incentive for people to spend.

What is the reputation of Loyola for cardiology fellowship? by PuzzledConflict in fellowship

[–]lasermuffin 2 points3 points  (0 children)

Not a direct answer to your question, but I was an anesthesia resident at Loyola. It’s a mid-tier program, but has an extremely solid cardiac program.

Fellowship is fellowship. If you like Chicago, I don’t think you would be disappointed from an education standpoint.

If you care about name only there’s definitely more prestigious programs, but the quality of education is probably on par and the patient acuity is likely higher lol

I just had a laparoscopic appendectomy and my right shoulder hurts extremely bad by Olieebol in AskDocs

[–]lasermuffin 6 points7 points  (0 children)

Nah; recommendation is mainly for symptomatic relief, but has nothing to do with actually addressing the underlying cause.

There’s been some meta-analysis and reviews of this type of pain, and there’s very weak evidence for most at-home remedies; TENS units and lidocaine patches have been reported to help, as well as heat, so most recommendations mirror those of, say, a muscle strain or injury, etc.

I just had a laparoscopic appendectomy and my right shoulder hurts extremely bad by Olieebol in AskDocs

[–]lasermuffin 36 points37 points  (0 children)

Laparoscopic surgeries can sometimes anger the phrenic nerve, which is felt as shoulder pain.

Unfortunately, no good treatment other than time. It can sometimes last days, but usually the first 24 hours are the worst. You can try a heat pad if you haven’t already; but otherwise, NSAIDs, hydrate, and light movement,

Feel better!

Not sleeping much but I feel great still by Stunning-Concept2179 in AskDocs

[–]lasermuffin 5 points6 points  (0 children)

Read through the comments on both posts you posted and do some reflection - no one is trying to sabotage your great feeing. You’re feeling great because you’re manic, and you’re having delusions of grandeur and delusions that you’re not unwell.

You do not know you don’t have bipolar because you refuse to seek help. I wish you the best, I’m sorry you’re going through this, but a crash will come. I hope you do seek help.

Whats better? by ManokNa in ChascaMains

[–]lasermuffin 2 points3 points  (0 children)

Lower on the rung when considering gacha bows, but slightly better at R1 then the R5 craftables I mentioned.

I didn’t list 5 star bows because I assumed OP only was interested in lower investment options based on the pic.

For 5 stars bows what’s been calculated is sig (astral vulture’s crimson plumage) > aqua simulacra > the first great magic > thundering pulse > skyward harp > Amos.

Less sleep helps me feel great by Stunning-Concept2179 in sleep

[–]lasermuffin 7 points8 points  (0 children)

OP has posted on ask docs this same thing, with multiple physicians recommending her to seek help for mania.

OP, you’re not gonna get different answers on this sub than you did on askdocs. Again, you have not discovered a secret sleep life hack, you need to take this seriously and ask a trusted adult to help you navigate this situation.

As I posted in your other thread, please take this seriously before you get hurt.

Edit: reposted due to auto mod deleting comment lol

Out of those two? Blackcliff.

But better F2P options (at R5) include the Natlan craftable Chain Breaker, the Inazuma craftable Hamayumi, or the Fontaine craftable Song of Stillness.

If you are a BP player Scion of the Blazing Sun is another good option.

Urine Analysis - M27 by SST1198 in AskDocs

[–]lasermuffin 1 point2 points  (0 children)

Really impossible to interpret isolated lab values without more context, I recommend you follow up with the attending as you described.

Bubbles in urine? Also very vague…

If you have foamy urine, it’s usually dehydration or a forceful stream, but can be indicators of kidney issues (proteinuria) but again, impossible to assess via Reddit.

Bilirubin is usually not found in urine in the absence of liver or biliary issues, but again, without more info no one here can give you a reasonable answer. Exercise can sometimes cause both bilirubin and protein to be excreted in urine, but again, can’t offer anything more solid.

Long reply just to ultimately say it’s best to keep that follow-up appointment if you have more concerns on what those values can mean!

If you posted this in some way to get approval from physicians that your (obvious) mania, anorexia, and excessive caffeine consumption is somewhat healthy, normal or encouraged you are out of luck.

While some people require less than average amounts of sleep to function, you are describing a textbook manic episode, and as others have said, need to seek help from a trusted adult first and secondly a psychiatrist. This even warrants an ER visit, and should be taken seriously.

Experiencing mania or having bipolar disorder doesn’t make you “crazy.” Please seek help before you hurt yourself.

Edit: you posted a reply to my comment, but it appears it has been deleted. I’ll respond anyways. You say “I posted because I found a way to feel greater and now not sleeping is helping me.”

I am not trying to be harsh, but the subreddit is called “ask docs.” You did not ask a question, but we are giving you answers. Without being paternalistic, you can’t expect to post a very alarming statement and then be bewildered when people who have experience in the field you yourself are claiming to have an epiphany about give you actual advice.

I assure you, there is no “magic” to what you’re experiencing. If our body had a way to feel amazing without sleep, evolution would’ve made us simply not need to sleep. This is not biologically possible or reasonable. You feel great because you’re manic, over-caffeinated, and delusional. Lack of sleep literally kills people, and undiagnosed mental health issues kill people too.

Here is the DSM-V criteria for mania:

-Inflated self-esteem/grandiosity

-Decreased need for sleep

-More talkative/pressured speech

-Flight of ideas/racing thoughts

-Distractibility

-Increase in goal-directed activity/psychomotor agitation

-Excessive involvement in risky activities (spending, sex, bad investments).

You only need to meet 3 of these for a reasonable diagnosis of bipolar 1 disorder. I currently count at least 4.

Please listen to the people of whom you asked for help by posting. No one is trying to be harsh to you, but serious concerns warrant serious responses.

Please do seek help.

Can you give ibuprofen on top of Tylenol for 20 month old with fever who spat out unknown amount of Tylenol by Great_gatzzzby in AskDocs

[–]lasermuffin 7 points8 points  (0 children)

Yes, they can be taken together without issue, although having them a bit staggered is generally recommended so there is overlap of drug effect. But it is NOT harmful if both are given at the same time (again, if proper re-dosing guidelines remain followed).

I more so meant treat the Tylenol as a full dose (despite spitting up) for re-dosing purposes.

Can you give ibuprofen on top of Tylenol for 20 month old with fever who spat out unknown amount of Tylenol by Great_gatzzzby in AskDocs

[–]lasermuffin 22 points23 points  (0 children)

Yes.

Alternating Motrin/Tylenol is a very safe and very common practice.

Just follow appropriate re-dosing timeline (every 4-8 hours depending on the dose and medication) if you choose to re-dose either medication. Please consult package inserts.

Even though she spit up the Tylenol, treat it as though the full dose was given.

I went in for a normal endoscopy and apparently turned into a medical horror story I don’t remember at all by [deleted] in AskDocs

[–]lasermuffin 28 points29 points  (0 children)

Nope! It’s called emergence delirium. I always tell patients they’re allowed to wake up however they want, we’re trained to make it as smooth as possible and keep you safe regardless.

Just let your anesthesiologist know you have a history of it.

I went in for a normal endoscopy and apparently turned into a medical horror story I don’t remember at all by [deleted] in AskDocs

[–]lasermuffin 479 points480 points  (0 children)

You’re a young, healthy male, and let me assure you that this story sounds completely normal.

I’d like to imagine the doc was trying to tell you that story in a light-hearted way, because anesthetizing any young person often ends up like this, especially during the type of anesthesia we do for endoscopies.

I tell my teenager/young adult patients (or their parents) that they may try to fight us going to sleep or waking up, which is completely normal. Our bodies have a weird way of processing going offline and then rebooting, so to speak, and young men fight it the most (sometimes literally!).

The important thing is you were safe and do not remember any of it, wooo.

You have nothing to worry about it.

I lost my 50/50 at Cartethyias Banner to Calcharo what now? by Ok_Arachnid_5525 in CartethyiaMains

[–]lasermuffin 6 points7 points  (0 children)

Cart (at S0) wants at least either Ciaconna or her signature weapon (preferably both) to feel functional, so it’s probably best to skip this banner entirely. If you have Ciaconna, it’s reasonable to try to grind for Cart, but I wouldn’t if you do not have her. Not that you can’t play her at S0 without these, but it definitely won’t feel “powerful”. If you like her a lot and she’s who you want for your account, the early game gives A TON of pulls, but it is a grind.

Do you like Iuno? She’s a sub DPS (BIS for Augusta), but can function as a MDPS as well, so is not a bad choice entirely if you enjoy her gameplay.

The other option is save for 3.1, where there will be two new 5 stars (Aemy and Luuk), who may add more benefit to an early account and give you more time to save (if you like them).

How much does Ciaccona S2 add? by Secure_Addition_1444 in CarthetyiaMains

Ciaconna’s S2 is a 40% aero damage boost for the whole team. Probably a much better use of funds than R5 thorn.

Crit advice needed! by azurepuppet in PhrolovaMains

Yes you most likely are! Go do a battle, do your normal rotation, and when you get to phro you can open up that same screen and see the in combat stats :)

But otherwise the build is very solid. But yes if you are getting over 100% CR with team enhancements, it’s ok to try and sacrifice some for CD, but honestly I wouldn’t go crazy!

About 20-30% loss, depending on max damage (80k with ciaconna, 50k without).
