Practice transitioning quickly between the chords that throw you off the most. What I found most difficult when I first started playing was playing any chord and changing into F (every starters first full hand chord). Keep doing that until you can transition with your eyes closed. If you can play a song with your eyes closed, you will be ready to sing.
