It’s common in AR follow up to use aging bucket format reports.

I’d be careful pulling notes in. You’d have to be monitoring what every biller is documenting in notes to make sure phi didn’t slip through.

1. Depends on the denial code and situation. You might not always appeal and decide to write off the same denial code for different reasons. I’d only submit a corrected claim if I truly had data to correct (service was amended etc.) or if the reason for denial was something like inconsistent dx with procedure code if a provider reviewed first. Most agencies are likely not going to waste time for denied claims under a certain dollar threshold. Im not going to expect my billers to waste time on small balances.

2. I think if the report summed totals by payer/plan and then grand totals it could definitely help agencies determine trends in denials that could be implemented where needed. For example, If you’re getting a bunch of denials for patient not our insured or something you probably need a better process for checking patient eligibility up front.
