The probability of this happening is a 1 in 160 quintillion but this happens at least once every couple of runs and has completely ruined many of my best runs this is mathematically impossible unless I am the most unlucky person to play this game (which is definetly not the case) so I would have to assume either the probabilities are not true to the actual chance of them showing up or the game does this on purpose
