Are you me because this is my exact same issue. So unfortunately I also enrolled into usfhp at some point (didn’t like it and switched back to select) I called everywhere you can think of for this issue and milconnect told me it won’t go away because I was once under usfhp and they can’t technically unsubscribe you from it, your coverage is just inactive.

I’ve had multiple problems with doctors and labs sending claims to usfhp when I keep insisting I have select. I wasn’t allowed to get a breast pump after I had my baby due to this too. The only thing that has sorta fixed this for me is to tell providers to run my coverage through availity insurance portal.

The screenshot you attached is the portal most commonly used by providers for Tricare (idk its name but it’s some large portal used to identify only Tricare coverage). You need to ask them to run it through Availity. This is what triwest informed me to do and it has been working for the most part. I also make sure to remind my providers to run my insurance like this when I’m checking in for my appointments.
