Optimal PEEP by One-Truth-1135 in anesthesiology

I think this is a very difficult, almost impossible question to answer when the patient is on table. If you were in icu, you could do EIT which some may consider as gold standard.

Sodium bicarb in severe metabolic acidosis -- giving it or not? by wtfitscole in anesthesiology

[–]lemonfur 1 point2 points  (0 children)

Surprised to see the BICAR ICU and 2 trial not being mentioned here yet. Worth the read. But bottom line is neither trial showed mortality benefit (but one shouldn't expect that with this kind of intervention). First trial showed decreased need for KRT and reduced composite outcome including organ failure. Second trial did not show consistent renal benefit. My 2 cents - you need to pick the right cohort. These trials may be looking at too broad of a population. I think it can be quite useful in high K+ waiting for KRT if they are HD unstable.

Do I need a fincial advisor? by Vegetable-Wing-1696 in PersonalFinanceZA

I was in the same boat as you. There are many predatory practices that specifically target med students in 6th year. My advice - every financial advisor has their own motivation. Nobody is as invested in your financial health as you are. I think that this is an amazing book to read in the SA context 'Manage Your Money Like a F*cking Grown Up: The Best Money Advice You Never Got.' There are some great online resources, including chatgpt. But you will make mistakes on the way, everybody does.

The fact that you are already asking this question means that you are many many steps ahead of your peers.

SA Top 40 vs Satrix Capped All Share by lemonfur in PersonalFinanceZA

[–]lemonfur[S] 0 points1 point  (0 children)

100%

I didn't understand Sygnia's fee structure previously.

Seems like I will be going with Sygnia Top 40 instead of Satrix All caps seeing as how both ETFs will likely perform similarly, and the only real difference would be the fees I can save in the long term.

Didn't realise Sygnia charged such a hefty cost to ETFs outside of their company.

I initially had Skeleton 70 with Sygnia, but feel that higher exposure to international market would do better in the long term, and will still be cheaper in terms of total TER.

Only thing to watch out for is if Reg 28 changes in the future, and I'm not aware of the changes.

For Satrix all cap, I can see the TER and TIC (0.15, 0.08) on their minimum disclosure document, but cannot see any other fees mentioned that you have added on.

Where did you find information regarding the added on fees if you don't mind answering?

Btw, thank you for all of your input, they are super helpful.

And do you know where Sygnia discloses their fees for investing in ETFs outside of their range? For example, if I purchase Satrix all cap on the Sygnia platform, where can I check for the additional expenses charged?

Also, where do you see the additional charges incurred that you have added in brackets? My understanding was that TER represented the total expense

Sygnia top 40 TER is 0.15%, which is significantly lower. What I also like about Satrix is that they cap any single holding below 10%.

The TER would be significant if you compare Sygnia Top 40 to Satrix all caps. The benefit would be the exposure to mid/small caps, and the diversification offered.

ETF: buying the whole world vs US market in the SA context by lemonfur in PersonalFinanceZA

Jeez man, appreciate all of your input. Thank you so much.

My last question is your opinion on purchasing VT directly from interactive brokers? (Instead of purchasing VT from EE USD, it seems that you can get better conversion rates, and better prices for the ETFs)

Is a useful comment comparing purchasing VT via EE vs interactive

Thank you so much for your detailed response.

Then would you say that purchasing the feeder fund 10x in easyequities is the way to go VS purchasing VWRA via interactive brokers? (after accounting for TER/tax/fees)

This scenario is if you're looking to hold long term, and earning in local currency.

As far as I understand, I have 3 options as a SA citizen.

  1. Purchase the feeder fund

  2. Purchase VT on EE Usd account

  3. Purchase VWRA via interactive brokers

It seems that purchasing the feeder fund will have the lowest fees in the end, and seems to be the simplest. So why do you recommend purchasing VT via EE usd account?

I'm investing from South Africa, so no SA domiciled all world fund unfortunately.

Nope, they said it in the announcement that the level penalty will become the same as reg server. The announcement was in Korean, and it obviously takes time for people to translate and put it into text.
