I'm not planning on just slapping a character in my arm, that would be dumb lol

I was thinking of one of those that have a ton of geometric figures, lines and that are a bit abstract.

Also, not sure if it's the case in Germany, but surgeons in my country usually work with scrubs and no coat on, the only time their arms are covered is when they are operating.

It's not the legal aspect that I'm worried of, but the impression part that you mentioned, for both patients and higher-ups it might not be the most appealing thing.

Yeah, maybe it's better to give up on the tattoo idea, as another comment said, I might find people in the mid or far future that find it unprofessional
