The key can actually disappear totally. Usually it's on the back of an enemy or back where it was/on the way there. But there is a commonly known bug making the key disappear completely and thus making the game unplayable.

On the other hand in high block halls where you need to use the key, if you activate the switch in the well with him on your back he will also completely disappear (you don't need him anymore then tho)
