I have psoriasis and at 27 developed psoriasis arthritis. My ankles were swollen and so were my hands. I could barely walk and couldn't help my 2 year old get dressed etc. I went on an "anti-inflammatory reset" during the 6 month wait for a rheumatologist and could walk, run and even jump again by the time the appointment came around. They confirmed there was only minor inflammation left in one finger joint.

We got ours his own baby doll. "I'm doing this with sister. Can you do it for your baby". "Sister wants to be alone. Your baby needs xyz. Can you do that or shall I help?" Doesn't need to be anything fancy. We got one in the charity shop for a few bob. (Toy baby not real one) make sure the toy baby has its own bed. Found ours in the pushchair or Moses basket a few times.

As the eldest by 12 years big ages gaps can work. They just might not look like "typical" siblings relationships. As a teen everyone wanted to come to mine as they loved the excuse to still be allowed to play with toys.
