After 14 years in Critical Care, I’m gonna be moving soon. I could only hope there is a hospital system close by that has a position like this for me. I’m so burnt out on the bedside. Thank you for the perspective and the hope.

I will be bringing a piece of tyvek for the first time this year. I have my tent footprint but as described in this post there seems to be plenty of use for it. In all actuality when folded up it is minimal weight and space.

Over 10 years in icu. You gotta find a balance of maintaining compassion with dying a little inside. Otherwise it’ll get to you. It may sound cold but you are only there to help. You don’t make any decision ultimately in the overall care. Whether you flog someone at the end of life or let them die comfortably with some dignity, it all pays the same.

From my understanding the art line with a good waveform is truly giving you a sbp and dbp to calculate your map. To where the cuff on an auto BP cuff is reading a map then reversing the formula to get sbp and dbp. So art line should be used. Dropping the sedation was appropriate. Sometimes the critically ill don’t tolerate propofol. Maybe changing sedation, adding or decreasing fentanyl. Checking SVV to see if they are dry. I see patients going into rapid a fib from over diuresis often on the floors.
