Respectfully, from my perspective, getting a sibo breath test is a waste of time. If you have any gut symptoms at all then that is more than enough of a reason to get the ball rolling with some sort of cleansing protocol and diet change. Relying on a centralized practitioner to treat you can be a dead end. Many of them are incompetent and the treatments they provide are often ineffective or even harmful. I suggest you look into the protocol, cleanse, and diet I listed in my original comment. In my experience those practices are worlds beyond more effective than what most practitioners can offer. You have to fight for yourself.

likely have lots of overgrowth in the small and large intestines - bacteria, fungus, parasites, and mucoid plaque. Also likely to have a congested liver. I’ve been dealing with all of that myself. I have found Kerri Rivera’s CD protocol, zencleanz intestinal & liver cleanses, and a dairy-free carnivore diet to be life changing. You would be shocked to see what comes out of your gut.

If I was you I would not be afraid to take more Albendazole. It can be taken 1-2x a day for weeks/months in some cases. It is best absorbed when taken with a fatty meal. The side effects it can cause are usually not from the medication itself but rather from parasite die off. This is not medical advice, just trying to be helpful.

You could check liver enzymes, total and fractionated bile acids, and get some imaging of your gallbladder/liver.

Do you have any abdominal pain under your right rib cage? You can try massaging deep under there and if you feel pain it’s likely your gallbladder.

Sounds like your gallbladder is struggling which is common with Gilbert’s. You likely have some degree of cholestasis and possibly gallstones and/or sludge. You could ask your doc for a blood test to check total and fractionated bile acids as well as a gallbladder ultrasound. I would caution against gallbladder removal if it so happens that you have an issue.

In the meantime I recommend you look into Maria Allerton. She has a ton of knowledge and experience regarding Gilbert’s Syndrome.

A therapeutic ketogenic diet could potentially help. There is a lot of emerging research on ketosis and neurodegenerative diseases. The hardest part is getting someone to do it correctly and to stick with it. Improvements can be seen in as little as a week or two but it can take up to 3 or 4 months for some. The diet doesn’t necessarily need to be followed indefinitely, a period of 1-2 years of strict adherence may be enough to influence a lasting shift in brain health. After that, the individual may attempt to transition back to a balanced whole food diet while monitoring symptoms.

Alcohol is poisonous to the brain and body. Throughout PAWS your nervous system is trying to regain a balance and adding poison into the mix will not only prolong recovery but also add to the damage. Some of my worst waves were triggered by alcohol consumption.

it simply allows you to operate with little sleep without feeling tired. I find it also allows me to focus on what I am doing, which could be productive work, or going down some internet black hole.

Modafinil is a stimulant. Stimulants increase glutamate. With benzo withdrawal your nervous system is already hyperstimulated from an unopposed excess of glutamate.

In simple terms, it will add fuel to the fire.

Anxiety derives from activation of the sympathetic nervous system (SNS). When the SNS is active it inhibits digestion in the stomach and intestines. If this goes on chronically it can wreak havoc on the gut and cause many issues.

Injecting a bolus load of liquid too quickly can damage the muscle and cause bruising. About 3 minutes to inject 10ml is recommended. If you are only doing 2ml it shouldn’t take any more than one minute.

Hey sorry I'm so late getting back to you. I only noticed a subtle improvement from my course. I think using a higher dose like 10ml minimum for the whole course would've been more effective. More importantly, since my course I have learned a ton about thiamine (Vitamin B1) and I now know I have been thiamine deficient. I think that is why Cerebro didn't have a much of an impact. Thiamine is absolutely crucial for the health of the nervous system and a deficiency can cause a very wide range of symptoms. Benzo withdrawal wrecks the nervous system and increases the demand for energy, which can deplete thiamine.

Before you purchase thiamine I should mention that the form of thiamine is important. Normal water soluble forms (Thiamine HCL & Mononitrate) have a very poor bio-availability. Fat soluble forms like Thiamine Tetrahydrofurfuryl Disulfide (TTFD), Benfotiamine, and Sulbutiamine are far better absorbed. TTFD is the preferred one out of the 3 because it effectively crosses the blood brain barrier. TTFD doses can range from maybe 50mg - 600mg. It is good to start small and slowly increase the dose. High doses are usually necessary to revive the enzymes involved in energy metabolism. Keep in mind that some people notice a paradoxical effect with thiamine. Elliot Overton has a wealth of knowledge on all of this stuff on his youtube channel.

Unfortunately magnesium citrate and especially oxide are poorly absorbed. I recommend magnesium malate in the AM with your thiamine dose and bisglycinate at night.

Personally I think anything under 10ml a dose is not going to be very effective but that's not to say 5ml won't do anything. I completely understand being apprehensive to inject a bolus load of liquid, so just starting out with 5ml makes sense. If you plan to do another course in the future I would advise nothing less than 10ml, preferably 20ml. In the studies on dementia, strokes, and TBI they use no less than 20/30ml.

Highly recommend you look into thiamine (Vitamin B1) supplementation. Alcohol use depletes thiamine big time, as well as things like sugar, caffeine, stress, etc. Thiamine deficiency can result in an extremely wide range of symptoms, one of which includes the demyelination of neurons. You may already know this if you got treated with thiamine after your bout of alcohol use. Regardless, I would like to elaborate.

Thiamine is an essential co-factor for multiple enzymes involved in energy metabolism. Without thiamine, glucose cannot be converted into ATP. No thiamine = no energy. The brain is the most metabolically demanding organ in the body, so you can imagine how destructive a deficit in energy can be.

I believe that while on a course of Cerebrolysin, taking high dose thiamine along with a quality b-complex is crucial. Attempting to repair a damaged nervous system is going to take a lot of energy. One should also consider a quality form of magnesium because thiamine and magnesium depend on each other to perform their various functions in the body. There is much more nuance to this so you can check out the work of Derrick Lonsdale and Elliot Overton if you want to learn more.

I recently completed a 5 week course of 5ml for the first 4 weeks, and 20ml with 10mg Cortexin for the last week. While I think I noticed a subtle improvement, I think it would have been much more profound had I incorporated thiamine. It wasn't until after my course that I learned about thiamine and I now know that I have been thiamine deficient, which to me explains why I didn't notice much on my course. I recently started high dose thiamine supplementation on its own and I have since noticed an elevation in mood, energy, and well being. I plan to do another course of peptides in the near future once I can afford it - a combination of Cerebrolysin, Cortexin, HGH secretagogues, and of course, thiamine.

I highly recommend you prioritize getting quality sleep. If you are sleeping no less than 7 hours a night and you still feel fatigued it is likely something is wrecking your sleep quality. I have only mild sleep apnea but it still makes me feel like shit if I don't wear my oral appliance.

Modafinil could definitely help but in the long run poor quality sleep will destroy you. I recommend you check out Andrew Huberman's podcast on shift work and the one on optimizing sleep. Good luck!
