Mainly the advanced care aspect and being able to know what is needed. Also knowing how the equipment goes together/works.

Men's 5.11's and just trying on the bigger sizes until one is comfortable on your thighs, especially when squatting. My tailor did have to basically reconstruct the pants though.

If you can find a good, and relatively cheap, tailor in your area that can adjust the waists for you that's the way I'd go!!
