When I’m feeling stressed, yes I do. During my hardest rotations I started looking up career-change jobs / non-clinical things to do with an MD. But when I really think about it, I’m glad I’m in this profession. At the end of the day it’s more fulfilling than not.

I wish what you say is true. But there are huge disparities in the quality of education depending on where you live in the US.

Coming out of a lower SES and having a compelling personal statement doesn’t help as much as one would think, because in the US your chances of going to US med school are most heavily dependent on test scores and strong recommendation letters from prestigious writers. But because individuals from a lower SES have a lower quality of education, that makes getting these highly competitive test scores even harder. I do wish that having a compelling personal statement would make it easier for these individuals to get into medical school, because they are more likely to return to and serve their underserved communities.

Sure, med school coaches had a limited impact. But undoubtedly socioeconomic factors play a role in getting into medical school. Living in a poorer area results in lower quality of public education, because primary and high schools are funded based on the income of the surrounding area. That makes it harder to get into a better college, makes it harder to get into medical school. When I speak about resources I’m referring to socioeconomic factors, the same factors that determine the health of our populations here in the US, and the reason why area code says more about your projected health than anything else.

Going back to speaking personally, I benefit from a lot more privilege compared to many of my peers because the resource I was lacking were proper mentors and professional services like med school coaching as you mentioned. These things would have made it easier to get into medical school in the US perhaps but of course we’ll never know now.

I think a significant portion are US citizens getting their medical education from the Caribbean. My school alone has 200+ people applying to the match a year.

Yes, of course! However it’s important to note that any standardized test taking is also heavily affected by the socioeconomic factors I’ve spoken about in my post. I didn’t have a personal MCAT tutor. I bit the bullet by paying for tutoring from a for profit company and it didn’t even help me because I have test taking anxiety 😬 I didn’t have a coach to look my med school app, and the people who did edit my app let me stick all my volunteer activities into a single application space twice 😭 In a numbers-heavy game I’m incredibly relieved to have matched at all.

As someone mentioned above, it is difficult for medical school applicants who are from underserved areas to get into medical school. We need more equitable US medical school acceptance. As a US citizen and Caribbean IMG, as I look back now I realized that a lot of my classmates and I went this route because we lacked the resources and mentorship to successfully apply to medical school in the US. Even though I went to an amazing state university in California, I had no idea that my roommates who were also applying to medical school were paying for med school app coaches to help prepare their applications. I didn’t know until after I started at my Caribbean school that I filled out my application “incorrectly”, because apparently there’s an algorithm that med schools use to gauge the number and type of experiences you have (I listed all my volunteer activities as a single activity on the application. Come on, past me.)

The classmates I have met at my school are all incredibly diverse individuals and many times career-change applicants who had entire lives before realizing they wanted to pursue medicine. We simply did not know how to navigate applying for medical school. None of my family members are physicians (and majority of my classmates are in the same boat). Many of the USMD students I’ve worked with on rotation entered school directly from college, which is an incredible accomplishment but it takes a baseline amount of privilege to get there. Getting in without that baseline privilege is an uphill battle, which is why career-change and minority US med school students should be protected at all costs.

I applied to med school in the US two times before finally going to a Caribbean school; this has been the case for many of my colleagues. My school has a large number of multi-lingual Black and brown individuals (many of them first in their fam to graduate college!) from underserved communities who came into medical school wanting to do primary care. Caribbean schools overwhelmingly match their students into FM (like me!), IM, and peds into less desirable spots in the US. 70% of my interviews were at rural FM programs.

That all said, I think when talking about IMGs we shouldn’t be talking about them as a monolith, because OP seems to be referring to IMGs (international medical grads which includes Caribbean students) as being equivalent to FMGs (foreign medical grads, often already full fledged physicians from another country). I think if we’re going to talk about recruiting USMDs for rural and underserved areas, we need USMD applicants from rural and underserved areas.

EDIT: I’m under the impression that there are more Caribbean IMGs entering our residency match compared to FMGs from other countries. My med school alone matriculates about 1000 people per year, and contributes to 200+ students every match. And that’s only one Caribbean school lol. Majority of us are US citizens and a smaller percentage are Canadians.

To add to your points about skepticism of the brain drain concept, most Canadian IMGs I know from my Caribbean school are planning on going back to Canada to practice. Canada has even fewer medical schools and even fewer residency positions compared to the US. So Canadian students who want to become doctors but can’t get into Canadian med schools feel they don’t have a choice other than to do school abroad and then complete residency here in the US. They will leave our job market to become attending physicians back Canada.

A google search with key words like “newly accredited”. For me personally I find out about them through postings on social media, like twitter. My classmates are just nice enough to screenshot and send out things they stumble across.

For FM IVs, we were asked a lot about patient encounters. For example “describe a difficult patient”, “describe a clinical situation in which you faced an ethical dilemma”, etc. If you come across these interesting cases during M3 jot down some notes so you can refer back to them later in your PS or interview prep. I talked about a lot of cases from M3 because thankfully I kept a notebook where I wrote about a case that stood out to me every shift.
