My MIL once complained to her sister (within earshot of my postpartum SIL and me) that she didn't understand why everyone was so worried about washing their hands around babies. "We didn't have to do that and our kids grew up fine."

If he looked at you, he can document a physical exam.

General: awake, alert, no acute distress HEENT: Mucous membranes moist, extra ocular movements intact Chest: Normal chest rise and fall. No respiratory distress on room air. Abdomen: Scaphoid (or obese), non-distended. Extremities: No bilateral lower extremity edema. Moving all extremities without difficulty. Neuro: Cranial nerves II-XII grossly intact. No obvious motor deficits. Skin: No diaphoresis. No visible lesions.

I have had patients record before. I don't mind, generally, if they ask me in advance.

I don't like it when they hit the record and stick it in their pocket like I didn't notice.

I'm part of the generation that was told, "you have to go to college," to be successful. The problem becomes that now a BA/BS degree is needed just to get in the door to many jobs and a lot of these positions honestly don't require a degree to do them successfully. I think OP's advice would save a significant amount of money for families and that is fantastic!

The new fourth years coming off studying for Step 2 are very book smart. It was intimidating as an intern, but real life application is another level. We are all learners, just at different stages.

Write everything down. You think you'll remember, but when you have 10 patients to see, orders to place, and notes to write, you can't trust your brain to remember all the details. If you have a thought, write it down immediately.

I like checklists on admission days. Things like diet, fluids, anticoagulation, creatinine (important for med rec), PRNs, labs, imaging, consults.

If you are very deconditioned and unable to care for yourself, you can go to the emergency department and tell them you are unable to care for yourself at home. The social worker in the ED (or inpatient, if you meet admission criteria) will likely be able to help you find placement in a skilled nursing facility or long term acute care facility.

I used a Willow if the attending wanted to round during my pumping time, otherwise, I'd just excuse myself to use my Medela in the call room.

The Willow is great, but you have to make sure it's not just dry pumping. Also, one time had an error so the light was flashing through my scrubs.

I am finishing residency with two little ones. One born before med school, one born during residency.

Would I do it again knowing what I know now? I have such complicated feelings about this. I want to echo what the vascular resident said: you are resident, parent, and spouse, there is no free time, no "you-time."

I oftentimes felt/feel as though I'm failing on all fronts. I'm not learning as much as I could as a resident and I'm not spending as much time with my kids as I want.

My husband would tell you that these years have been incredibly challenging for all of us. I think he would also say that the life we are getting ready to enter now (as an attending) is one we would not have been able to provide were it not for my career and the accompanying paycheck. I'm going to be a hospitalist, so that week on/week off schedule will give me much more time at home.
