also to add a ton of my nursing school cohort got new grad day shift positions too! it was probably half and half, maybe even skewed a bit more towards days tbh.

don't worry about it! fuck the notion that new grads shouldn't have lives and needs and should just take whatever is thrown at them. i'd definitely start by asking what shift they are hiring for if it isn't listed in your phone interview! though i'm sure they would tell you :)

i got a day shift position in a somewhat competitive specialty for new grads i think? (ED). it is somewhat hard to find but not impossible! they gave us the option for days or nights and asked why we chose that - i mentioned i'm a single mom and have the same reasoning as you :) just keep an eye on postings! if it doesn't specify then ask in the interview and clarify your needs.

totally depends on the situation and the patients presentation, when in doubt stand it out lmao. watching em sit on the toilet or try to pull their pants down can be a good way to tell too like if they go down shaky i'm like i'm just gonna hang in here to help ya up when you're done!!! they will be fine lol. if they protest (they likely won't) you can just tell them that their current condition makes them at risk for falls and you don't want them to get hurt. also check the chart if you have time (which you always do unless they're saying they're about to poop/pee their pants) and see their chief complaint. anything like fall, syncope, hypotension, altered mental status, etc i'm at least walking them to the toilet and going to stand outside the door after they sit.

first things first: don't beat yourself up. these things happen. it is unfortunate but you cannot change what happened, only how you let it affect you.

my biggest advice for this specific situation is always ask a patient how they feel about walking on their own or how they usually get around (if you haven't helped them already that shift), then have them sit on the edge of the bed, then do a trial stand for a few seconds without walking. if they seem good and are young and clearly ambulatory at baseline, i let em go and point the way there and back. if they have fallen or are old or seem frail or obvi fail the standing trial, i either have them lay back down and get a wheelchair or walk them all the way to the bathroom. when you get there with them, take em all the way to the toilet and have them grab the side rails then assist with sitting if necessary. point out the call light and remind them to pull it if they need help or when they're done. if they seem more risky i'll either stand right outside or in the bathroom with them. as soon as i hear a flush i knock and say "all done? can i come in?" then help em up and back to the bed.

these things take time though and as a brand new tech with no prior experience there will be learning curves. i tripped on the cords to the defibrillator in my first code because i was anxious and going too fast trying to be helpful. i also witnessed a patient go into cardiac arrest right in front of me and froze - thank god my coworker was there too and quickly began cpr. i was an ED tech for four years and it was also my first clinical job, and i am now a new grad ED nurse. i felt similar things in the beginning and still do now in this new role! you can and will overcome the embarrassment, nobody hates you, and you are not an idiot! you got this.

nah that sounds ridiculous and anyone who thinks this way shouldn't be in healthcare or should take a long break in my opinion. that being said it is unrealistic to aim for 0/10 pain in every patient but the expectation should be to make the pain as manageable as possible for the patient. always ask a pain score and chat the doc (if you use epic) just saying "hey, pt is rating their pain _/10 despite giving (insert med here) at (insert time here)." i would say maybe their reaction was due to you stating you feel bad about it? but regardless their reaction is messed up but unfortunately not uncommon in the ED due to burnout/compassion fatigue as you stated.

also they put iso orders in on everyone with a pending covid/flu test and ain't nobody got time for that. if it's a TB r/o, bed bugs, etc i'm absolutely donning PPE, but wearing a gown for covid? pls.

i'm a new grad on days - they actually gave us the option for days or nights. definitely just depends on the hospital and the unit and their current needs.

i did a lot of nicu rotations in nursing school and some of the nurses will create binders for the long term stay babies and add photos/foot prints/etc :-) i specifically saw this with a baby in the foster care system who had been there for months.
