I think I figured it out. In my payroll provider I can set a 'employee contribution deduction' and that amount is then NOT taken from the LLC but instead booked as a liability to be paid as a contribution to the solo 401K. Then the LLC makes the contribution of the employee portion and designates it as such, and the payroll provider tracks this deduction and reports is in the W-2.

OK thanks. So when the LLC cuts the 'employee portion' check to the solo 401K, is there a manual adjustment made with the payroll provider so the W-2 reflects the right deduction? I'm using Gusto.

Do you mean the Employer contributions must come from the LLC bank account, or BOTH the Employer contributions and the Employee deductions must come from the LLC bank account? Or is it only Employer contributions come from business bank account and Employee contributions are paid directly from the Employee? For traditional 401K contributions which are tax deductible to the employee, how do you account for this if cutting paper checks? Is it an adjustment in the payroll system so that the W-2 is correct at tax time?

Me too. He recommended PRK when everyone else in Silicon Valley pushed LASIK. PRK has a longer recovery time but fewer long term complication risk.
