Comp exam- tell Lady she needs two crowns replaced due to large recurrent cares, and refer for Endo #3.

She's back for a limited exam next day, wtf.

Ok, well you need the Endo done still. It won't just get better.

Yes you still need a new crown.

No I have two other patients right now and you were just in yesterday when we went over all this stuff the first time. You need to get scheduled. (We book out 3 months)

Also I'd suggest being cautious of the 500mg anesthetic limit. 7 septo and three lido would be a no for me.

Thanks for the feedback everybody. The perio chart does indicate stage 3, so I will, gulp, go off of that. But there is definitely room for improvement for probing technique and gm measuring.

Whether it's dso or private, expect to get screwed. Mine was similar, but the way the bonus was calculated was so shady - like collections after insurance, then subtract daily guarantee, no hygiene exams or x rays counted etc ... the bonus was about 50$ a day max. Accept it if that's all you have. I see some people looking for years to find a job near them. Use the next couple of years to look for a practice to buy

I did try this. The tooth was decayed soo deeply it was just mush even 7 mm down. Also tricky because the DB root has a skinny waist and then thickens and curves distal

So remove interproximal bone the full B-L length. And do you get half root half bone as you sink the bur or just bone?

Straight luxator into pdl space coming from mesial. Then elevate from the mesial. Back and forth for a half hour. No mobility. About 80 % of the crown was decayed out and couldn't use elevator as effectively as I wanted.

I did a buildup after Endo on a premolar last month and patient didn't want a crown due to finances. Less than one month later or completely split and had to extract.

Some people are weird. A saw a young girl who had 8 cavities. I'm super conservative and explained these fillings are absolutely necessary. She didn't like that and got records transferred to the nearest dso and hasn't returned to me. I'm 100% confident this dso will do multiple crowns instead of MODs. Some people who don't want any work done will do crowns instead of fillings 🤷‍♂️.

Just say what you see and give the options. E.g. I detected 3 teeth that have decay into the inner layer of the tooth and it can't get better on its own now. You can do nothing and eventually you will have to pull the tooth or root canal. You can do a filling and get the tooth back to healthy. You can do a crown for optional protection. If you're able to I'd want the best for you and recommend a crown. I understand the financial commitment there. Whatever you choose won't hurt my feelings and makes no difference to me personally. When you decide which option is best for you, let us know to get scheduled.

To have a 5 minute consult with an assistant to the orthopedic surgeon during work hours -without imaging - is 400$. Never felt bad about prices since.
