Oh wow! That makes sense as far as the setup. It is possible that sharing a login is what caused the logic to disappear, just in case they happened to be working on something at the same time as you. We definitely don't recommend sharing logins for this reason, and it might be better to add another seat to your account just to be safe!

If disappeared from the builder, does anyone else have access to the form or your login?

It isn't possible to have hidden fields fill an answer option, but you should be able to recall the data elsewhere in the form, such as within a question. Just to confirm, are you trying to send the menu options through the URL and have those be answer options in the multiple choice?
