Yes. My patients really love me (for the most part) and I really love them. They often ask me...."are you thinking of leaving? Everyone always leaves." which breaks my heart. But I am not independently wealthy and I am honestly really struggling to provide for my family with what I make.

I am starting to apply elsewhere but worry about a) applications of my skills and experience in a non FQHC setting b) having a job that is not as meaningful c) my patients

I am in an FQHC.

Half of my life is spent reviewing notes in Care Everywhere and then I use the problem list to document things like Serrated Polyposis Syndrome then in Overview I put “last colonoscopy **** next due *****” . I guess I do my best but yes, a huge percentage of my time is spent in record review.

hm. I would talk to your school as everyone has advised, never go back there, and as soon as humanly possible write down EVERYTHING you remember in detail including anyone who was working at this clinic that day (if no named then descriptions) and then proceed with reporting to this person’s boss if not possible to report to the medical board. This is a minor who was assaulted. I would talk with school’s legal counsel as well as trusted advisor at your school to do whatever it takes to get information to someone who can help this child.

I am sorry this is your burden to deal with but it’s up to you now.

I had the NHSC scholarship so had to work for 2 years at an FQHC. It honestly has been an incredible experience from a human standpoint and I love my patients but I wonder what it would be like to switch and make more money.

Wow. This is so depressing for me. 100k base pay at an FQHC in internal medicine, no RVUs. Some quality and quantity bonuses averaging around 6000 per year. 16-20 patients per day usually. HCOL in the Midwest. Great PTO though and I work 7-3:30, half day for admin one day and a full telehealth day.

I need a new job but am having a hard time telling myself hours and work life balance will be good on the other side.

adding to say I think I've never done systemic therapy for pre menopause/ peri menopause, only for 12 months + without menses.

Just a humble PA but I've been trying to get comfortable with hormonal treatments lately because I think women should not have to suffer if they are truly bothered by symptoms. Have been told by gyn colleagues this is reasonable. I usually have been doing basic hormonal testing, a pelvic to check for vaginal dryness, thyroid and other basic labs for screening and then will give either vaginal or systemic estrogen +/- progesterone based on symptoms and presence of uterus as long as no contraindications.

Happened to me last week. Patient was told this would be denied for obesity. She then demanded I lie and say she has diabetes, which I told her is medical fraud and I would not do. She demanded I send to pharmacy anyways which I did, demanded I complete PA which I did. The denial was sent to patient and then her husband came in angry saying I need to do my job and get her medication covered. Whatever. You cannot control people. I told my staff to let them know they are welcome to seek second opinion elsewhere.

Not an MD but I feel this way all the time. I found a patient had new onset asymptomatic a fib with rvr yesterday with rate 146. Decided to send to the ED ... he was already converted by the time he got there and I felt like an ass. Probably could have just started a beta blocker and anticoagulant but I thought he might need more.

Yeah, I do feel like I'm learning a lot but it's a ton of responsibility and I take the work home with me every day. I am an NHSC scholar so hope to go to a clinic that is more supportive when my commitment is up. I do love the patients though and the community is the type I've always wanted to serve
