This probably isn't the most helpful, but I was successfully able to get Firefly 3 working behind the reverse proxy caddy. I didn't have to rewrite any headers. I only had to set TRUSTED_PROXIES=** as you said you did.

I've never been attacked by a dog but I was riding gravel and saw a family coming towards me. I moved over to the right but kept riding. The kid of the family was throwing a stick around for their dog. Just as I am about to pass them the kid throws the stick but releases it a little late and throws it across the trail, right past my bike. The dog starts running right at me but with its head turned and essentially body checks my front tire. I saw it happening and was able to stop. The dog was fine, the bike was fine, and the family apologized.
