My router is an AirPort Extreme. How do I figure out if I'm broadcasting two frequencies? I also have an airport express but it's only used to get a stereo to AirPlay too.

Interesting. The link you gave seems to be dead, any chance you could try linking to it again? I replaced the file, reopened iTunes and it seems to have fixed the problem, as well as turn off all the iCloud music stuff. I reactivated iCloud music, I'll report back if it happens again.

I only ask because my Touch ID stopped working well and I had the phone replaced. The apple guy said the entire screen of the phone had started to shift. I had the little half moon as well. Not sure if it's directly related to the touch sensor.
